胸腔镜肺楔形切除术对早期非小细胞肺癌患者围术期情况及预后的影响

Effect of Thoracoscopic Wedge Resection of Lung on Perioperative Condition and Prognosis of Patients with Early Non-small Cell Lung Cancer

目的探讨胸腔镜肺楔形切除术对早期非小细胞肺癌(NSCLC)患者围术期情况及预后的影响。方法选取80例早期NSCLC患者,按随机数字表法分为两组,各40例。对照组予以解剖性肺叶切除术,观察组行胸腔镜肺楔形切除术,观察至术后3个月。对比两组围术期相关指标、免疫功能、并发症。结果观察组术中出血量、术后前3 d引流量较对照组少,手术、术后胸管引流、漏气、住院时间较对照组短,并发症发生率为7.50%,较对照组低25.00%,有统计学差异(P<0.05);术前两组Th1、Th2、Th17细胞水平相比,无统计学差异(P>0.05);术后观察组Th1细胞水平较对照组高,Th2、Th17细胞水平较对照组低,有统计学差异(P<0.05)。结论相较于解剖性肺叶切除术,胸腔镜肺楔形切除术在早期NSCLC患者治疗中效果更佳,具备出血量少、并发症发生率低、术后恢复更快等优势,还能够有效保护患者免疫功能,安全可行,应用价值较高。

Objective To investigate the effect of thoracoscopic wedge resection of lung on the perioperative situation and prognosis of patients with early non-small cell lung cancer(NSCLC).Methods 80 patients with early NSCLC were selected and divided into 2 groups according to random number table method,40 patients in each group.The control group was treated with anatomical lobectomy,and the observation group was treated with thoracoscopic lung wedge resection,and the observation lasted for 3 months.The perioperative related indexes,immune function and complications were compared between the 2 groups.Results The amount of intraoperative bleeding and drainage in the first three days after operation in the observation group was less than that in the control group.The operation and postoperative thoracic tube drainage,air leakage and hospital stay were shorter than those in the control group.The incidence of complications was 7.50%,was lower than that in the control group,25.00%,with statistical difference(P<0.05);There was no statistical difference in the levels of Th1,Th2 and Th17 cells between the 2 groups before operation(P>0.05);After operation,the level of Th1 cells in the observation group was higher than that in the control group,and the level of Th2 and Th17 cells in the observation group was lower than that in the control group,with statistical difference(P<0.05).Conclusion Compared with anatomical lobectomy,thoracoscopic lung wedge resection is more effective in the treatment of early NSCLC patients,with less bleeding,low complication rate,faster postoperative recovery and other advantages.It can also effectively protect the patient's immune function,safe and feasible,and has higher application value.
