沙库巴曲缬沙坦联合螺内酯对慢性心力衰竭患者心功能及B型利钠肽的影响

Effects of Sacubitril Valsartan Combined with Spironolactone on Cardiac Function and B-type Natriuretic Peptide in Patients with Chronic Heart Failure

目的探讨沙库巴曲缬沙坦联合螺内酯在慢性心力衰竭(congestive heart failure,CHF)患者治疗中的应用价值。方法选取2021年1—12月梁山县人民医院收治的CHF患者100例,按随机数字表法分为对照组和研究组,各50例。对照组采用螺内酯联合美托洛尔治疗,研究组采用沙库巴曲缬沙坦联合螺内酯治疗。对比两组临床疗效、心功能、心室重构指标、B型利钠肽(BNP)、不良反应。结果研究组治疗总有效率(96.00%)高于对照组(82.00%),差异有统计学意义(χ^(2)=5.005,P<0.05);治疗后,研究组左室射血分数、左室重构指数、每搏心输出量高于对照组,左心室舒张末期内径、左心室收缩末期内径、左心室质量指数、左室壁应力、BNP低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论沙库巴曲缬沙坦联合螺内酯能够有效改善CHF患者心功能,降低BNP水平,逆转心室重构,安全可靠。

Objective To explore the application value of sacubitril valsartan combined with spironolactone in the treatment of patients with chronic heart failure(CHF).Methods 100 CHF patients admitted to Liangshan County People's Hospital from January to December 2021 were selected and randomly divided into a control group and a study group,with 50 patients in each group.The control group was treated with spironolactone combined with metoprolol,while the study group was treated with salbutamol combined with spironolactone.Compared the clinical efficacy,cardiac function,ventricular remodeling indicators,B-type natriuretic peptide(BNP),and adverse reactions between two groups.Results The total effective rate of the study group(96.00%)was higher than that of the control group(82.00%),the difference was statistically significant(χ^(2)=5.005,P<0.05).After treatment,the left ventricular ejection fraction,left ventricular remodeling index,and cardiac output per stroke in the study group were higher than those in the control group,while the left ventricular end diastolic diameter,left ventricular end systolic diameter,left ventricular mass index,left ventricular wall stress,and BNP in the study group were lower than those in the control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Sacubitril valsartan combined with spironolactone can effectively improve cardiac function,reduce BNP levels and reverse ventricular remodeling in CHF patients,and is safe and reliable.
