乳腺癌患者外周血miR29a的表达及临床意义

摘要 目的探讨乳腺癌患者外周血miR29a的表达及临床意义。方法选取2020年1月—2022年12月牡丹江市第二人民医院收治的30例乳腺癌患者为乳腺癌组、30例乳腺良性增生患者为乳腺良性病变组,另选取同期50例健康志愿者为对照组。采用实时荧光定量PCR法(qRT-PCR)测定并比较3组外周血miR29a的表达,采用Spearman相关性分析miR29a表达与乳腺癌病理特征的相关性,并采用受试者工作特征(ROC)曲线分析外周血miR29a表达对乳腺癌的诊断效能。结果乳腺癌组患者外周血miR29a表达量高于乳腺良性病变组和对照组,差异有统计学意义(P<0.05)。不同乳腺癌TNM分期、分化程度、肿瘤直径及淋巴结转移患者miR29a表达量比较,差异有统计学意义(P<0.05);不同年龄、病理类型乳腺癌患者外周血miR29a表达量比较,差异无统计学意义(P>0.05)。Spearman相关性分析显示,外周血miR29a表达量与乳腺癌患者TNM分期、肿瘤直径、淋巴结转移呈正相关(P<0.05),与分化程度呈负相关(P<0.05)。ROC曲线分析显示,外周血miR29a表达量诊断乳腺癌的曲线下面积(AUC)为0.960(95%CI:0.917~1.000),最佳截断值为0.55,预测敏感度、特异度分别为83.3%、96.3%。结论乳腺癌患者外周血miR29a表达水平显著升高,且与临床病理特征密切相关,可作为临床诊断乳腺癌的生物学指标之一。 Objective To investigate the expression and clinical significance of miR29a in peripheral blood of patients with breast cancer.Methods 30 patients with breast cancer admitted to Mudanjiang Second People´s Hospital from January 2020 to December 2022 were selected as the breast cancer group,and 30 patients with benign breast hyperplasia were selected as the breast benign lesion group,and 50 healthy volunteers during the same period were selected as the control group.Real-time quantitative fluorescence PCR(qRT-PCR)was used to determine and compare the expression of miR29a in peripheral blood of the three groups.Spearman correlation was used to analyze the correlation between miR29a expression and pathological features of breast cancer,and receiver operating characteristics(ROC)curve was used to analyze the diagnostic efficacy of miR29a expression in peripheral blood for breast cancer.Results The expression level of miR29a in peripheral blood of breast cancer group was higher than that of breast benign lesion group and control group,and the difference was statistically significant(P<0.05).There were statistically significant differences in miR29a expression level among patients with different TNM stages,differentiation degree,tumor diam‑eter and lymph node metastasis(P<0.05).There was no statistically significant difference in miR29a expression level in peripheral blood of breast cancer patients of different ages and pathological types(P>0.05).Spearman correlation analysis showed that miR29a expression in peripheral blood was positively correlated with TNM stage,tumor diameter and lymph node metastasis of breast cancer patients(P<0.05),and negatively correlated with differentiation degree(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of peripheral blood miR29a expression in the diagnosis of breast cancer was 0.960(95%CI:0.917~1.000),the optimal cut-off value was 0.55,and the predictive sensitivity and specificity were 83.3%and 96.3%,respectively.Conclusion The expression level of miR29a in peripheral blood of patients with breast cancer is significantly increased,which is closely related to clinicopathological features,and can be used as one of the biological indicators for clinical diagnosis of breast cancer.
