超声造影剂注射用六氟化硫微泡致过敏性休克1例

A case of anaphylactic shock caused by ultrasound contrast agent sulfur hexafluoride microbubbles for injection

摘要 1例59岁男性乙型肝炎肝硬化患者为明确肝占位性病变性质行肝脏超声造影检查,静脉推注六氟化硫微泡约20 s后,患者出现表情淡漠、焦躁不安、血压下降、乳酸增高等表现,考虑为造影剂导致的过敏性休克。立即停用该药,迅速给予抗过敏、抗休克、补液、扩容等治疗,38 min后患者过敏性休克症状明显缓解,生命体征恢复正常。 To clarify the nature of the hepatic space-occupying lesions,a 59-year-old male patient with hepatitis B virus related cirrhosis underwent liver contrast-enhanced ultrasonography.About 20 s after intravenous injection of sulfur hexafluoride microbubbles,the patient showed apathy,restlessness,decreased blood pressure and increased lactic acid,which was considered to be anaphylactic shock caused by contrast agents.We stop the drug immediately,and give anti-allergy,anti-shock,fluid replacement and volume expansion treatments quickly.After 38 minutes,the symptoms of anaphylactic shock of the patient were significantly relieved,and his vital signs returned to normal.
