经尿道前列腺等离子切除术对良性前列腺增生患者排尿功能及性功能的影响

Effect of Transurethral Plasmakinetic Resection of Prostate on Urination Function and Sexual Function in Patientswith Benign Prostatic Hyperplasia

目的:探讨经尿道前列腺等离子切除术(PKRP)对良性前列腺增生(BPH)患者排尿功能及性功能的影响。方法:选取2019年1月—2022年2月徐州市贾汪区人民医院泌尿外科收治的BPH患者72例为观察对象,根据随机数字表法分为对照组与研究组,各36例。对照组实施经尿道前列腺电切术治疗,研究组实施PKRP治疗。比较两组患者手术相关指标、术后排尿功能、性功能、前列腺症状。结果:研究组手术时间、膀胱冲洗时间、导管留置时间、住院时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.001)。手术后,两组患者残余尿量(RUV)均降低,最大尿流率(Qmax)均升高,差异有统计学意义(P<0.05);研究组RUV低于对照组,Qmax高于对照组,差异有统计学意义(P<0.001)。手术后,两组患者国际勃起功能指数问卷表-5(IIEF-5)评分均升高,国际前列腺症状量表(IPSS)评分均降低,差异有统计学意义(P<0.05);研究组IIEF-5评分高于对照组,IPSS评分低于对照组,差异有统计学意义(P<0.001)。结论:PKRP治疗BPH的临床效果显著,对患者造成的损伤更小,可促进其术后的排尿功能及性功能恢复。

Objective:To investigate the effect of transurethral plasmakinetic resection of prostate(PKRP)on urination function and sexual function in patients with benign prostatic hyperplasia(BPH).Methods:A total of 72 patients with BPH admitted to Department of Urology Surgery,People's Hospital of Jiawang District of Xuzhou from January 2019 to February 2022 were selected as the observation objects.They were divided into control group and study group according to random number table method,with 36 patients in each group.The control group was treated with transurethral electroresection of the prostate and the study group was treated with PKRP.The operation related indexes,postoperative urination function and sexual function,and prostate symptoms in the two groups were compared.Results:The operation time,bladder irrigation time,catheter indwelling time,and hospital stay in the study group were shorter than those in the control group,and the intraoperative blood loss in the study group was less than that in the control group,and the difference was statistically significant(P<0.001).After surgery,the residual urine volume(RUV)decreased and the fastest flow rate(Qmax)increased in two groups,and the difference was statistically significant(P<0.05);The RUV in the study group was lower than that in the control group,and Qmax in the study group was higher than that in the control group,and the difference was statistically significant(P<0.001).After surgery,the scores of the 5-item version of the International Index of Erectile Function(IIEF-5)increased and the scores of the International Prostate Symptom Scale(IPSS)decreased in two groups,and the difference was statistically significant(P<0.05);The IIEF-5 score in the study group was higher than that in the control group,and the IPSS score in the study group was lower than that in the control group,and the difference was statistically significant(P<0.001).Conclusion:PKRP has a significant clinical effect in the treatment of BPH,with less damage to patients,and it can promote the recovery of postoperative urination function and sexual function.
