穴位贴敷联合麻杏石甘汤加减治疗老年慢性阻塞性肺疾病急性加重期的临床疗效观察

Clinical Efficacy Observation of Acupoint Sticking Combined with Modified Maxing Shigan Decoction in Treatment ofAcute Exacerbation of Chronic Obstructive Pulmonary Disease in Elderly Patients

目的:研究老年慢性阻塞性肺疾病急性加重期(AECOPD)应用穴位贴敷联合麻杏石甘汤加减治疗的临床效果。方法:选取2018年1月—2021年12月湖北省恩施土家族苗族自治州建始县人民医院肺病科收治的AECOPD患者280例为观察对象,根据随机数字表法分为对照组与研究组,各140例。对照组采用常规西药治疗,研究组在对照组基础上给予穴位贴敷联合麻杏石甘汤加减治疗。比较两组患者中医证候积分、肺功能、炎性因子水平、治疗效果。结果:治疗后,研究组胸闷、喘息、咳痰、哮鸣音评分低于对照组,差异有统计学意义(P<0.001)。治疗后,研究组患者各项肺功能指标水平高于对照组,差异有统计学意义(P<0.001)。治疗后,研究组各项炎性因子水平低于对照组,差异有统计学意义(P<0.001)。研究组治疗总有效率高于对照组,差异有统计学意义(P=0.010)。结论:穴位贴敷联合麻杏石甘汤加减治疗AECOPD疗效显著,可有效减轻患者临床症状,降低炎性因子水平,改善肺功能。

Objective:To study the clinical effect of acupoint sticking combined with the modified Maxing Shigan Decoction in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)in elderly patients.Methods:A total of 280 patients with AECOPD admitted to Department of Pulmonology,People's Hospital of Jianshi of Enshi Tujia and Miao Autonomous Prefecture in Hubei Province from January 2018 to December 2021 were selected as the observation objects.They were divided into control group and study group according to random number table method,with 140 cases in each group.The control group was treated with conventional western medicine and the study group was treated with acupoint sticking combined with the modified Maxing Shigan Decoction on the basis of control group.The TCM syndrome scores,lung function,inflammatory factor levels,and treatment effects in two groups were compared.Results:After treatment,the scores of chest distress,gasping,expectoration and wheezing in the study group were lower than those in the control group,and the difference was statistically significant(P<0.001).After treatment,the levels of lung function indexes in the study group were higher than those in the control group,and the difference was statistically significant(P<0.001).After treatment,the levels of inflammatory factors in the study group were lower than those in the control group,and the difference was statistically significant(P<0.001).The total effective rate of treatment in the study group was higher than that in the control group,and the difference was statistically significant(P=0.010).Conclusion:Acupoint sticking combined with modified Maxing Shigan Decoction in the treatment of AECOPD has a significant effect,which can effectively alleviate the clinical symptoms of patients,reduce the level of inflammatory factors and improve lung function.
