米氮平联合柴胡桂枝干姜汤加减治疗抑郁症的效果观察

Observation on Effect of Mirtazapine Combined with Modified Chaihu Guizhi Ganjiang Decoction in Treatment of Depression

目的:探讨米氮平联合柴胡桂枝干姜汤加减治疗抑郁症的效果。方法:选取2022年3月—2023年1月正定县正定杨建斌中医综合诊所收治的54例抑郁症患者作为研究对象,根据随机数字表法分为观察组和对照组,各27例。对照组给予米氮平治疗,观察组在对照组基础上应用柴胡桂枝干姜汤加减治疗。比较两组中医证候积分、抑郁情况、不良反应发生情况。结果:治疗前,两组大便秘结、失眠多梦、不思饮食、腹胀嗳气、神疲乏力、胸闷心烦、精神抑郁积分比较,差异无统计学意义(P>0.05);治疗后,两组各证候积分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗前,两组汉密尔顿抑郁量表(HAMD)评分比较,差异无统计学意义(P>0.05);治疗1、2、4、6周后,观察组HAMD评分均低于对照组,差异有统计学意义(P<0.001)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:米氮平联合柴胡桂枝干姜汤加减治疗抑郁症的效果显著,可改善患者临床症状与抑郁情况,安全性较高。

Objective:To investigate the effect of mirtazapine combined with modified Chaihu Guizhi Ganjiang Decoction in treatment of depression.Methods:A total of 54 patients with depression treated in Yang Jianbin Traditional Chinese Medicine Comprehensive Clinic in Zhengding County from March 2022 to January 2023 were selected as the study subjects.They were divided into control group and observation group according to random number table method,with 27 cases in each group.The control group was treated with mirtazapine and the observation group was treated with modified Chaihu Guizhi Ganjiang Decoction on the basis of the control group.The TCM syndrome scores,depression,and adverse reactions in two groups were compared.Results:Before treatment,there was no significant difference in the scores of constipation,insomnia,dreaminess,poor appetite,abdominal distension and belching,fatigue,chest tightness and upset,and mental depression between the two groups(P>0.05).After treatment,the scores of each syndrome in the two groups were lower than those before treatment,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in Hamilton Depression Scale(HAMD)scores between the two groups(P>0.05).After 1,2,4 and 6 weeks of treatment,the HAMD scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.001).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Mirtazapine combined with modified Chaihu Guizhi Ganjiang Decoction has a significant effect in the treatment of depression,it can improve the clinical symptoms and depression of patients,and has a high safety.
