陈旧性前交叉韧带损伤患者的膝关节三维运动学分析

Three-dimensional kinematic analysis of knee joint in patients with old anterior cruciate ligament injury

目的探讨陈旧性前交叉韧带损伤患者步行时患膝的膝关节三维步态变化情况。方法30名陈旧性前交叉韧带损伤患者为实验组,30名健康成人为对照组,利用膝关节三维运动测量分析系统记录2组膝关节的运动周期(步频、最大步长、最小步长、平均步长)和三维六自由度(屈伸角、内外翻角、内外旋角、前后位移、上下位移、内外位移)平均值并相互比较。结果同对照组比较,实验组的步频、屈伸角、前后位移与健康成人比较差异具有统计学意义(P<0.05),而最大步长、最小步长、平均步长、内外翻角、内外旋角、上下位移、内外位移与健康成人比较差异无统计学意义(P>0.05)结论与健康成人的膝关节的运动周期和三维六自由度平均值相比,陈旧性前交叉韧带损伤患者的步频变快、屈伸角减小、前后位移增大。

Objective To investigate the changes of three-dimensional gait of knee joint in patients with old anterior cruciate ligament injury during walking.Methods 30 patients with old anterior cruciate ligament injury were selected as the experimental group and 30 healthy adults as the control group.The average value and knee joint motion cycle(step frequency,maximum step,minimum step and average step)and three-dimensional six degrees of freedom(flexion and extension angle,internal and external valgus angle,internal and external rotation angle,anterior and posterior displacement,upper and lower displacement,internal and external displacement)were recorded and compared with each other by knee join three-dimensional motion measure analysis system.Results Compared with the control group,there were significant differences in step frequency,flexion and extension angle,anterior and posterior displacement between the experimental group and healthy adults(P<0.05).There was no significant difference in maximum step size,minimum step length,average step size,valgus angle,internal and external displacement,internal and external displacement between the experimental group and healthy adults(P>0.05).Conclusions Compared with the motion cycle and three-dimensional six-degree-of-freedom average of the knee joint of healthy adults,patients with old anterior cruciate ligament injury have faster step frequency,decreased flexion-extension angle,and increased anterior-posterior displacement.
