Latarjet手术对陈旧性肩关节前脱位的治疗效果分析

Analysis on the effect of Latarjet surgery on old anterior dislocation of shoulder joint

目的本文主要探讨切开复位,利用lartajet手术治疗陈旧性肩关节前脱位的效果。方法本院自2017-2021年收治陈旧性肩关节脱位患者5例,平均脱位时间9.2周(5~16周)。均伴有肱骨Hill-sachs损伤,肩胛盂前缘压缩性缺损。治疗采用切开松解复位,利用喙突截骨lartajet手术并修复肱骨近端骨缺损的方法。评估肩关节功能手术前后情况。结果术后均获得随访,平均7~8月(5~13月),5例患者肩关节稳定,未出现再次完全脱位。疼痛症状减轻,关节活动度明显改善,外展>90度,外旋>60度。VAS评分明显降低。UCLA评分从术前16±5.4分提高到术后25±2.8分(P=0.04)。结论陈旧性肩关节脱位临床上十分少见,治疗困难。本研究方法,能有效恢复肩关节稳定性,治疗效果满意。

Objective This paper mainly discusses the effect of open reduction and Latarjet operation on old anterior dislocation of shoulder joint.Methods Five patients with old anterior dislocation of shoulder joint were admitted to our hospital from 2017 to 2021.The average dislocation time was 9.2 weeks(5-16 weeks),who had Hill-sachs injury of humerus and compression defect of the leading edge of glenoid.The treatment was performed by open release reduction,coracoid osteotomy Latarjet surgery and repair of proximal humerus bone defect.Evaluate the effect of shoulder joint function before and after the operation.Results All the patients were followed up for an average of 7-8 months(5-13 months).The shoulder joint of 5 patients was stable and no complete dislocation again.The pain symptoms were reduced,and the range of motion of the joint was significantly improved,with abduction>90 degrees and external rotation>60 degrees.VAS score decreased significantly.UCLA score increased from 16±5.4 before the operation to 25±2.8 after the operation(P=0.04).Conclusions Old dislocation of shoulder joint is very rare and difficult to treat in clinic.This method can effectively restore the stability of shoulder joint and the therapeutic effect is satisfactory.
