关节镜下改良outside-in穿刺缝合技术修复半月板撕裂的临床观察

目的探讨关节镜下改良outside-in法治疗半月板撕裂的临床疗效。方法选择本院2020年8月-2021年12月收治的86例半月板撕裂患者,随机分为对照组和观察组,对照组采用传统outside-in法、观察组采用改良outside-in法,分别缝合半月板并观察术中、术后康复及并发症等情况。结果2组患者术前VAS、Lysholm评分、IKDC评分无明显差异(P>0.05);较之对照组,观察组平均每针缝合时长、半月板缝合失效率、神经血管损伤及术后感染情况、术后VAS评分明显偏低(P<0.05);术后Lysholm评分、IKDC评分、临床总有效率明显升高(P>0.05)。结论改良outside-in法治疗膝关节半月板撕裂具有操作简单、手术时间短、定位准确、创伤小、恢复快等优点,疗效确切。

Objective To discuss the clinical effect of repairing meniscus tear with modified outside-in puncture suture technique under arthroscopic.Methods 86 patients with meniscus tear admitted to our hospital from August 2020 to December 2021,who were divided into the control group and the observation group randomly.The control group used the traditional outside-in method and the observation group used the modified outside-in method to suture the meniscus and observe intraoperative,postoperative rehabilitation and complications.Results There was no significant difference in VAS,Lysholm and IKDC score between the two groups(P>0.05);Compared with the control group,the average time of suture per stitch,meniscus suture failure rate,neurovascular injury and postoperative infection and postoperative VAS score in the observation group were significantly lower(P<0.05);Postoperative Lysholm score,IKDC score and total clinical effective rate significantly increased(P>0.05).Conclusions Compared with the traditional outside-in method,the modified outside-in method has definite clinical effect and the advantages of simple operation,short operation time,accurate positioning,small trauma and rapid recovery in the treatment of meniscus tear of knee joint.
