高位前入路与加用后内侧入路定位胫骨隧道重建后交叉韧带的比较

Comparison of posterior cruciate ligament of locating the tibial tunnel reconstruction with a high anterior approach and adding a posteromedial approach

目的探讨膝前高位双入路与加用后内侧入路的两种镜下定位胫骨隧道的方法重建后交叉韧带后的临床结果。方法回顾性分析2018年4月-2021年3月在本院PCL重建63例患者的临床资料。29例采用双入路,34例采用三入路。对比2组手术时间、术后并发症、关节功能评分。结果2组患者均成功完成手术,围手术期未发生并发症。双入路组手术时间少于三入路组(P<0.05),但2组术后随访关节功能评定差异无统计学意义(P>0.05)。2组VAS、Lysholm和IKDC评分较术前显著改善(P<0.05);2组间膝关节活动范围无统计学意义(P>0.05)。结论前方高位双入路与三入路两种方法定位后交叉韧带胫骨骨道重建后交叉韧带均能获得满意临床疗效。

Objective To compare the clinical outcomes of posterior cruciate ligament(PCL)of locating the tibial tunnel reconstruction under arthroscopic with two kinds methods of high anterior approach and adding posteromedial approach.Methods A retrospective analysis was conducted on the data of total of 63 patients who received arthroscopic PCL reconstruction in our hospital from April 2018 to March 2021.29 patients were treated with dual approach,while the remaining 34 patients were treated with 3-portal technique.It compared the operation time,postoperative complications,knee function score.Results All the 63 patients in the two groups successfully completed the operation,and no complications occurred in the perioperative period.The operation time in dual approach group was less than that in anterior double portals group proved significantly superior to the three portal group in 3-portal group(P<0.05).However,there was no statistical significance in the difference of postoperative joint function between the two groups(P>0.05).The VAS,Lysholm,and IKDC scores improved significantly which compared with those before the operation(P<0.05).There was no statistical significance in knee motion range in two groups(P>0.05).Conclusions Two kinds methods of high anterior dual approach and 3-portal technique,which can obtain satisfactory clinical curative effect for posterior cruciate ligament of locating the tibial tunnel reconstruction.
