宣白承气汤对慢性阻塞性肺疾病合并胃肠功能障碍患者胃肠道症状及胃肠道激素水平的影响

Effects of the Xuanbai Chengqi decoction on gastrointestinal symptoms and gastrointestinal hormone levels in COPD patients with gastrointestinal dysfunction

目的:探究宣白承气汤对慢性阻塞性肺疾病(Chronic Obstructive Pulmonary Disease,COPD)合并胃肠功能障碍患者胃肠道症状及胃肠激素的影响。方法:以2021年6―12月在成都中医药大学附属医院接受治疗的78例COPD合并胃肠功能障碍患者采用随机数字表法分为对照组和观察组,各39例。对照组在COPD常规治疗基础上予以枸橼酸莫沙必利分散片治疗,观察组在常规治疗方案上加用宣白承气汤治疗。比较两组患者治疗前及治疗14 d后肺功能指标[第1秒用力呼气容积(Forced Expiratory Volume in 1 Second,FEV_(1))、肺呼吸时第1秒用力呼吸容积与用力肺活量之比(FEV_(1)/Forced Vital Capacity,FVC)]、中医证候积分及胃肠功能指标[胃动素(Motilin,MTL)、胃泌素(Gastrin,GAS)]水平。结果:治疗14 d后较治疗前,两组患者肺功能指标(FEV_(1)、FEV_(1)/FVC)水平、部分胃肠功能指标(MTL)水平均有上升(P均<0.05),且观察组高于对照组(P均<0.05);治疗14 d后较治疗前,两组患者中医证候积分、部分胃肠功能指标(GAS)水平均有下降(P均<0.05),且观察组低于对照组(P均<0.05)。结论:宣白承气汤能够改善COPD合并胃肠功能障碍患者肺功能、缓解胃肠道症状、调节胃肠激素状态,于患者预后有利。

Objective:To explore the effect of the Xuanbai Chengqi decoction(宣白承气汤)on gastrointestinal symptoms and gastrointestinal hormone levels in COPD patients with gastrointestinal dysfunction.Methods:A total of 78 COPD patients with gastrointestinal dysfunction treated in Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from June 2021 to December 2021 were divided into the control and the observation groups,with 39 cases in each group.The control group was treated with mosoril citrate based on conventional COPD treatment;the observation group was treated with the Xuanbai Chengqi decoction on the basis of the conventional treatment regimen.The pulmonary function indexes[forced expiratory volume in one second(FEV_(1)),ratio of forced expiratory volume in one second to forced vital capacity(FEV_(1)/FVC)],TCM syndrome score and gastrointestinal function indexes[motilin(MTL),gastrin(GAS)]were compared between the two groups before treatment and 14 days after treatment.Results:After 14 days of treatment,compared with those before treatment,the levels of pulmonary function indexes(FEV_(1),FEV_(1)/FVC)and partial gastrointestinal function index(MTL)index in the two groups were increased(P<0.05),and the observation group was higher than the control group(P<0.05).After 14 days of treatment,compared with those before treatment,the TCM syndrome score and partial gastrointestinal function index(GAS)in the two groups were decreased(P<0.05),and the observation group was lower than the control group(P<0.05).Conclusion:The Xuanbai Chengqi decoction can improve pulmonary function,relieve gastrointestinal symptoms and regulate gastrointestinal hormone status,which is beneficial to patient prognosis.
