三黄固本汤联合来氟米特治疗对系统性红斑狼疮患者中医证候积分、疾病活动度及免疫球蛋白的影响

Effects of the Sanhuang Guben decoction plus leflunomide on TCM syndrome score,disease activity and immune globulin in patients with systemic lupus erythematosus

目的:探究三黄固本汤联合来氟米特治疗对系统性红斑狼疮(Systemic Lupus Erythematosus,SLE)患者中医证候积分、疾病活动度及免疫球蛋白的影响。方法:从2019年3月―2021年6月于青岛大学附属威海市立第二医院接受治疗的SLE患者中随机选取92例,采用随机数字表法分为观察组和对照组,各46例。对照组患者予以来氟米特等药进行治疗,观察组则在对照组来氟米特等药物的治疗方案基础上加用三黄固本汤。记录治疗3个月后两组患者治疗效果,比较治疗前及治疗3个月后两组患者中医证候积分、免疫球蛋白[免疫球蛋白A(Immune Globulin A,IgA)、免疫球蛋白G(Immune Globulin G,IgG)、免疫球蛋白M(Immune Globulin M,IgM)]。对比治疗前、治疗1个月后、治疗3个月后两组患者系统性红斑狼疮疾病活动度量表(Systemic Lupus Erythematosus Disease Activity Index,SLEDAI)评分。结果:治疗3个月后,观察组患者治疗有效率高于对照组(P<0.05)。治疗3个月后较治疗前,两组患者各项中医证候积分、免疫球蛋白(IgA、IgG、IgM)水平均有降低,且观察组低于对照组(P<0.05)。治疗1个月后、治疗3个月后较干预前,两组患者SLEDAI评分均有上升,且观察组高于同时间段对照组(P<0.05)。结论:三黄固本汤联合来氟米特治疗SLE相较于单独使用来氟米特具有更加可观的治疗效果,还可帮助SLE患者缓解发热、皮疹等临床症状,有效控制疾病活动状况,调节免疫因子,增强患者免疫能力,具有临床推广及使用价值。

Objective:To explore the effect of the Sanhuang Guben decoction(三黄固本汤)plus leflunomide on TCM syndrome score,disease activity and immune globulin in patients with systemic lupus erythematosus(SLE).Methods:From March 2019 to June 2021,92 SLE patients who were treated in Weihai Second Municipal Hospital Affiliated to Qingdao University were randomly selected and divided into the observation group and the control group by random number table method,with 46 cases in each group.The patients in the control group were treated with leflunomide and other drugs,while the observation group was treated with the Sanhuang Guben decoction on the basis of the treatment plan in the control group with drugs such as leflunomide.After 3 months of treatment,the therapeutic effects in the two groups were recorded.The TCM syndrome scores,immune globulin[immunoglobulin A(IgA),immunoglobulin G(IgG),Immunoglobulin M(IgM)]before and after 3 months of treatment were compared.The systemic lupus erythematosus disease activity index(SLEDAI)score was compared between the two groups before treatment,after 1 month of treatment,and after 3 months of treatment.Results:After 3 months of treatment,the effective rate of patients in the observation group was higher than that in the control group(P<0.05).After 3 months of treatment,compared with those before treatment,the scores of TCM syndrome and the levels of immunoglobulins(IgA,IgG,IgM)in the two groups were decreased,and the observation group was lower than the control group(P<0.05).After 1 month of treatment and 3 months after treatment,the SLEDAI scores in the two groups were increased,and the observation group was higher than the control group during the same time period(P<0.05).Conclusion:The Sanhuang Guben decoction plus leflunomide has a more considerable therapeutic effect than leflunomide alone in the treatment of SLE.It can also help SLE patients relieve clinical symptoms such as fever and rash,and effectively control disease activity.It can regulate immune factors and enhance patients’immunity,which has clinical promotion and application value.
