临床药师参与1例替雷利珠单抗致免疫相关性不良反应的药学实践

Clinical Pharmacists Participated in the Pharmaceutical Practice of an Immune-related Adverse Event Induced by Tislelizumab

目的探讨临床药师参与抗肿瘤治疗,可促进患者用药更安全、有效。方法通过参与1例替雷利珠单抗引起免疫相关性不良反应的全过程,准确辨别致敏药物,分析不良反应可能发生的机制,并适时对患者和护士进行用药宣教,从临床药学角度为临床用药提供建设性意见。结果不良反应由雷利珠单抗引起,临床药师的建议被医生部分采纳,不良反应得到及时发现和控制,避免了对患者的进一步伤害,患者好转出院。结论临床药师参与肿瘤患者化疗过程,对药物调整的决策、不良反应的处置以及患者用药依从性的宣教都有积极推动作用,保障患者用药更安全。

OBJECTIVE To explore a working mode that promotes safer and more accurate medication for clinical pharmacists involved in pharmaceutical treatment of adverse drug reactions in antineoplastic therapy.METHODS By participating in the treatment of one case of immune-related adverse reactions caused by Tislelizumab,the possible causes,mechanisms and preventive measures for the adverse reactions were analyzed.Constructive suggestions from pharmacist were adopted for clinical medication,and other pharmaceutical care were timely carried out for patients and nurses.RESULTS The adverse drug reactions were caused by Tislelizumab.Through the pharmacists and clinicians′efforts,the adverse reactions were identified and resolved,resulting in continuous improvement in patient care.CONCLUSION Clinical pharmacists participate in the chemotherapy process of cancer patients,and play an active role in promoting rational medication,disposing of adverse reactions and increasing the population′s awareness of drugs,effectively ensuring the medical safety of patients.
