社区药学照护规范的建立与实践

Establishment and Practice of Community Pharmaceutical Care Norms

目的通过建立社区药学照护规范,药师直接及负责地提供与使用药物有关的照护,目的是让患者达到一个明确的治疗目标,进而提升患者的生活质量,从而探索药学服务的新模式。方法通过从药学照护的发展历程、药学照护应建立的认知及照护理念等角度出发,明确照护内容、流程以及照护目标,真正达到让患者的药物治疗都符合适应症、有效、安全、经济及提高用药依从性。结果药学照护规范的建立,推动了药学学科建设,促进了药师服务水平的提升,避免用药错误,保障患者用药安全。结论通过药学照护的实践与发展,推动集团内药学服务的蓬勃发展,更为我国药学服务工作进一步发展提供参考。

OBJECTIVE Through the establishment of community pharmaceutical care norms,pharmacists directly and responsibly provide care related to the use of drugs,with the aim of enabling patients to achieve a clear treatment goal,thereby improving the quality of life of patients,and exploring a new model of pharmaceutical care.METHODS From the perspective of the development process of pharmaceutical care,the cognition that pharmaceutical care should establish,and the concept of care,clear the care content,process,and care objectives,so as to truly make the drug treatment of patients conform to the indications,effective,safe,economic,and improve drug compliance.RESULTS The establishment of pharmaceutical care norms has promoted the construction of pharmaceutical discipline,promoted the improvement of pharmacist service level,avoided medication errors,and ensured the safety of patients′medication.CONCLUSION Through the practice and development of pharmaceutical care,it can promote the vigorous development of pharmaceutical care in the group and provide reference for the further development of pharmaceutical care in China.
