游离桡动脉掌浅支皮瓣修复指体皮肤缺损及桥接再植的临床效果分析

摘要 目的探讨游离桡动脉掌浅支皮瓣修复指体皮肤缺损及桥接再植的临床效果。方法回顾性分析2020年2月—2022年1月在苍南县人民医院及台州骨伤医院治疗的74例指体皮肤缺损及桥接再植患者的临床资料,按照皮瓣修复方法的不同分为研究组(39例)和对照组(35例),对照组给予静脉皮瓣修复术治疗,研究组给予游离桡动脉掌浅支皮瓣修复术治疗,比较两组临床疗效、治疗相关情况、感觉功能恢复程度及并发症发生率。结果研究组治疗优良率(94.87%)较对照组(71.43%)更高,差异有统计学意义(χ^(2)=7.462,P<0.05)。与对照组比较,研究组术中出血量更少,皮瓣存活率更高,创面愈合时间更短,差异有统计学意义(t=4.861、9.488、6.287,P<0.05)。研究组感觉功能恢复程度优于对照组,差异有统计学意义(Z=2.082,P<0.05)。研究组并发症发生率(2.56%)较对照组(22.86%)更低,差异有统计学意义(χ^(2)=5.338,P<0.05)。结论指体皮肤缺损及桥接再植患者采用游离桡动脉掌浅支皮瓣修复术治疗,能够提高临床疗效,减少术中出血量,提高皮瓣存活率,缩短创面愈合时间,改善感觉功能恢复情况,减少并发症。 Objective To investigate the clinical effect of free radial artery superficial palmar branch flap in repairing finger skin defect and bridging replantation.Methods Retrospective analysis the clinical data of 74 patients with finger skin defects and bridge replantation who were treated in Cangnan County People's Hospital and Taizhou Bone Injury Hospital from February 2020 to January 2022.According to the different methods of skin flap repair,they were divided into the study group(39 cases)and the control group(35 cases).The control group was treated with venous flap repair,and the study group was treated with free radial artery superficial palmar branch flap.The clinical efficacy,treatment related conditions,the degree of sensory function recovery and the incidence of complications were compared between the two groups.Results The treatment excellence rate in the study group(94.87%)was higher than that in the control group(71.43%),and the difference was statistically significant(χ^(2)=7.462,P<0.05).Compared to the control group,the study group had less intraoperative bleeding,higher flap survival rate,and shorter wound healing time,and the difference was statistically significant(t=4.861,9.488,6.287,P<0.05).The recovery of sensory function in the study group was better than that in the control group,and the difference was statistically significant(Z=2.082,P<0.05).The incidence of complications in the study group(2.56%)was lower than that in the control group(22.86%),and the difference was statistically significant(χ^(2)=5.338,P<0.05).Conclusion The use of free radial superficial branch flap for skin defect and bridge regrafting can improve clinical efficacy,reduce intraoperative blood loss,increase the survival rate of the flap,shorten the wound healing time,improve the recovery of sensory function and reduce complications.
