MSCT检查联合凋亡相关基因PDCD5检测对肺癌的诊断价值 被引量:1

The diagnostic value of MSCT combined with the detection of apoptosis-related gene PDCD5 in lung cancer

摘要 目的探讨多层螺旋CT(MSCT)检查联合凋亡相关基因程序性细胞死亡分子5(PDCD5)检测对肺癌的诊断价值。方法选取本院收治的119例肺癌患者作为观察组,选取同期肺部良性病变患者100例作为对照组。对所有受试者均行MSCT扫描,并检测血清中凋亡相关基因PDCD5的表达,同时分析肺癌患者PDCD5表达与MSCT征象之间的相关性。绘制受试者工作特征(ROC)曲线,分析两者单独诊断及联合诊断肺癌的诊断效能。结果观察组血清PDCD5表达显著高于对照组,组间差异具有统计学意义(P<0.05);肺癌患者在肿瘤大小、空泡征、分叶征、实性结节、支气管截断征、胸膜凹陷征、胸腔积液的血清PDCD5表达比较,组间差异无统计学意义(P>0.05);而肺癌患者在淋巴结转移的血清PDCD5表达比较,组间差异具有统计学意义(P<0.05);Pearson检验发现肺癌患者血清中凋亡相关基因PDCD5的含量与淋巴结转移呈负相关(r=-0.462,P<0.05);ROC曲线显示,血清凋亡相关基因PDCD5与MSCT扫描诊断评估肺癌的AUC分别为0.768、0.803,而两者联合诊断时AUC为0.911,此时敏感度、特异度及准确率最高分别为84.51%、79.17%及82.35%。结论MSCT扫描联合凋亡相关基因PDCD5检测在肺癌诊断过程中,其特异度、敏感度及准确率较高,可为肺癌的诊断与治疗提供确切指导。 Objective To explore the diagnostic value of multi-slice spiral CT(MSCT)combined with the detection of apoptosis-related gene PDCD5 for lung cancer.Methods 119 patients with lung cancer treated in our hospital were selected as the study group,and 100 patients with benign lung lesions were selected as the control group in the same period.MSCT scanning was performed on all subjects,and the expression of apoptosis related gene PDCD5 in serum was detected.At the same time,the correlation between PDCD5 expression and MSCT signs in patients with lung cancer was analyzed.The receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic efficacy of both individual and combined diagnosis of lung cancer.Results The expression of serum PDCD5 in the study group was significantly higher than that in the control group(P<0.05);There was no significant difference in the expression of serum PDCD5 in tumor size,vacuole sign,lobulation sign,solid nodule,bronchial truncation sign,pleural depression sign and pleural effusion(P>0.05);There was significant difference in the expression of serum PDCD5 in patients with lung cancer with lymph node metastasis(P<0.05);Pearson test showed that the content of apoptosis related gene PDCD5 in serum of patients with lung cancer was negatively correlated with lymph node metastasis(r=-0.462,P<0.05);The ROC curve showed that the AUC of serum apoptosis related gene PDCD5 and MSCT was 0.768 and 0.803,respectively,while the AUC of combined diagnosis was 0.911.At this time,the sensitivity,specificity and accuracy were the highest,with 84.51%,79.17%and 82.35%,respectively.Conclusion MSCT scan combined with apoptosis-related gene PDCD5 monitoring has high specificity,sensitivity and accuracy in the diagnosis process of lung cancer,which can provide precise guidance for the diagnosis and treatment of lung cancer.
