荆花胃康胶丸联合质子泵抑制剂三联疗法治疗幽门螺杆菌相关胃溃疡的研究

Jinghua Weikang capsule combined with proton pump inhibitor triple therapy in the treatment of Helicobacter pylori-related gastric ulcer

目的探究荆花胃康胶丸联合质子泵抑制剂(PPI)三联疗法对幽门螺杆菌(Hp)阳性胃溃疡患者的疗效,以期为临床治疗决策的制定提供参考。方法选取2018年7月—2020年7月四川省人民医院东篱医院收治的Hp感染伴胃溃疡患者96人作为研究对象,采用随机数字表法分为对照组及研究组,每组48例。对照组患者给予铋剂四联方案进行治疗,研究组患者给予荆花胃康胶丸组联合PPI三联方案进行治疗。比较2组患者Hp根除率、疗效及不良反应发生情况。结果停药28 d时,研究组患者Hp根除率为82.61%,高于对照组的77.77%,但差异无统计学意义(P>0.05)。治疗结束10 d时,研究组患者痊愈12人,显效17人,有效15人,无效2人;对照组患者痊愈5人,显效12人,有效20人,无效8人。研究组患者疗效优于对照组,且差异有统计学意义(P<0.05)。治疗前,2组患者上腹胀、上腹痛、纳差等症状的症状指数比较,差异无统计学意义(P>0.05);治疗结束时及停药后28 d时,2组患者上腹胀、上腹痛、纳差等症状的症状指数均优于治疗前,且差异均有统计学意义(P<0.05)。结论荆花胃康胶丸联合PPI三联疗法治疗Hp阳性胃溃疡,与铋剂四联疗法相比疗效更好。该方案可作为Hp阳性胃溃疡患者的治疗方案之一,尤其对于不宜应用铋剂患者,疗效显著,安全性高。

Objective This paper attempts to investigate the therapeutic effect of Jinghua Weikang capsule combined with proton pump inhibitor(PPI)triple therapy on Helicobacter pylori(Hp)-positive gastric ulcer to provide evidence for clinical treatment decision-making.Methods Ninety-six patients with Hp infection and gastric ulcer admitted to Sichuan Provincial People's Hospital,Dongli Hospital from July 2018 to July 2020 were selected as the study participants.They were assigned to the control group and the study group by the random number table,with 48 cases in each group.The patients in the control group received bismuth quadruple therapy,and the patients in the study group were treated with the Jinghua Weikang capsule combined with PPI triple therapy.Hp eradication rate,therapeutic efficacy,and adverse reactions were compared between the two groups.Results The eradication rate of Hp was 82.61%in the study group,which was higher than that(77.77%)in the control group,but without significant difference(P>0.05).At the end of treatment,12 patients in the study group were cured,17 were markedly effective,15 were effective and 2 were ineffective;5 patients in the control group were cured,12 were markedly effective,20 were effective and 8 were ineffective.The therapeutic efficacy in the study group was significantly superior to the control group(P<0.05).Before treatment,there was no significant difference in the symptom index of upper abdominal distension,upper abdominal pain,anorexia,and other symptoms between the two groups(P>0.05).At the end of treatment and 28 days after drug withdrawal,the symptom index of upper abdominal distension,upper abdominal pain,anorexia,and other symptoms in the two groups were significantly superior to those before treatment(P<0.05).Conclusion The Jinghua Weikang capsule combined with PPI triple therapy is more effective than bismuth quadruple therapy in the treatment of Hp-positive gastric ulcers.This regimen can be used as one of the treatment options for patients with Hp-positive gastric ulcers,especially for patients who are not suitable for bismuth,with marked response and high safety profiles.
