甲状腺过氧化物酶抗体测定在自身免疫性甲状腺疾病中的应用效果

Application of thyroid peroxidase antibody in autoimmune thyroid disease

目的探讨甲状腺过氧化物酶抗体(TPOAb)测定在自身免疫性甲状腺疾病中的应用效果。方法选取2019年1月至2020年6月本院收治的180例甲状腺病变患者作为研究对象,根据疾病类型分为Graves甲亢组(n=61)、淋巴细胞性甲状腺炎组(n=21)、桥本氏甲状腺炎组(n=54)、亚急性甲状腺炎组(n=15)、单纯性甲状腺肿组(n=17)、分化型甲状腺癌组(n=12),另选取30名健康体检者作为对照组,均给予血清TPOAb浓度测量,比较各组TPOAb水平与阳性率,根据ROC曲线分析TPOAb诊断自身免疫性甲状腺疾病的效果。结果Graves甲亢组、淋巴细胞性甲状腺炎组、桥本氏甲状腺炎组与亚急性甲状腺炎组、单纯性甲状腺肿组、分化型甲状腺癌组、健康对照组TPOAb水平与阳性率比较差异有统计学意义(P<0.05);不同自身免疫性甲状腺疾病间的TPOAb水平与阳性率比较差异无统计学意义;ROC曲线下cut-off值为33.40 IU/ml时,TPOAb诊断自身免疫性甲状腺疾病的灵敏度为83.0%,特异度为90.0%。结论通过检测TPOAb诊断自身免疫性甲状腺疾病的作用较好,可为疾病的诊治提供参考。

Objective To investigate the application of thyroid peroxidase antibody(TPOAb)in autoimmune thyroid diseases.Methods 180 patients with thyroid diseases admitted to our hospital from January 2019 to June 2020 were selected as the study subjects,including Graves hyperthyroidism(n=61),lymphocytic thyroiditis(n=21),Hashimoto's thyroiditis(n=54),subacute thyroiditis(n=15),simple goiter(n=17),differentiated thyroid cancer(n=12),and 30 healthy people were selected as the control group,the serum TPOAb concentration was measured in each group,the TPOAb level and positive rate of each group were compared,and the effect of TPOAb in diagnosing autoimmune thyroid disease was analyzed according to ROC curve in each group.Results There were significant differences in TPOAb levels and positive rate among Graves hyperthyroidism group,lymphocytic thyroiditis group,Hashimoto's thyroiditis group,subacute thyroiditis group,simple goiter group,differentiated thyroid cancer group and healthy control group(P<0.05);There was no significant difference in TPOAb levels and positive rate among different autoimmune thyroid diseases;When the cut off value under ROC curve was 33.40 IU/ml,the sensitivity and specificity of TPOAb in diagnosing autoimmune thyroid disease were 83.0%and 90.0%respectively.Conclusion The detection of TPOAb plays a good role in the diagnosis of autoimmune thyroid diseases and can provide reference for the diagnosis and treatment of diseases.
