穴位按压联合自由体位及适度保护会阴法在初产妇阴道分娩中的应用价值

Application value of acupoint pressing combined with free position and moderate perineal protection in vaginal delivery of primipara

目的探讨穴位按压联合自由体位及适度保护会阴法在初产妇阴道分娩中的应用价值。方法选取2019年1月至2021年1月于江西省妇幼保健院进行阴道分娩的71例初产妇作为研究对象,随机分为A组(n=36)和B组(n=35例)。A组采用自由体位及适度保护会阴法助产,B组在A组基础上采用穴位按压方式助产。比较两组会阴疼痛程度、分娩情况及新生儿结局。结果B组会阴疼痛程度低于A组,差异有统计学意义(P<0.05);两组产钳助产率比较差异无统计学意义;B组产后出血、剖宫产、会阴侧切发生率均低于A组,差异有统计学意义(P<0.05);两组新生儿体质量比较差异无统计学意义;B组新生儿阿氏(Apgar)评分高于A组,差异有统计学意义(P<0.05)。结论穴位按压联合自由体位及适度保护会阴法能减轻初产妇阴道分娩时的疼痛,降低剖宫产率、会阴侧切率及产后出血率,并提高新生儿Apgar评分,值得临床推广应用。

Objective To investigate the application value of acupressure combined with free position and moderate perineal protection in vaginal delivery of nulliparous women.Methods A total of 71 cases of nulliparous women who underwent vaginal delivery in Jiangxi Provincial Maternal and Child Health Hospital from January 2019 to January 2021 were randomly divided into group A(n=36)and group B(n=35).Group A adopted the free position and moderate perineal protection method of midwifery,and on this basis,group B adopted acupressure midwifery on the basis of group A.Perineal pain,delivery,and neonatal outcomes were compared between the two groups.Results The perineal pain degree of group B was lower than that of group A,and the difference was statistically significant(P<0.05);there was no significant difference in the rate of forceps assisted delivery between the two groups,the incidence of postpartum hemorrhage,cesarean section and episiotomy in group B was lower than those in group A,and the difference was statistically significant(P<0.05);there was no significant difference in body weight between the two groups;and the Apgar score of newborns in group B was higher than that in group A,and the difference was statistically significant(P<0.05).Conclusion Acupressure combined with free position and moderate perineal protection can reduce the pain of vaginal delivery,and the rate of cesarean section,perineal incision and postpartum hemorrhage,and improve the Apgar score of newborns,it is worthy of clinical promotion and application.
