摘要
目的探究超声造影联合超声弹性成像技术鉴别诊断乳腺肿块良恶性的临床价值。方法选取2020年2月至2021年2月于本院诊断的79例乳腺肿块患者作为研究对象,所有患者均进行超声弹性成像及超声造影检查,以病理检查结果为准,比较超声弹性成像、超声造影及联合检测的诊断结果。结果超声弹性成像联合超声造影的灵敏度、准确度及特异度(93.75%、92.41%、92.06%)均高于超声弹性成像(50.00%、68.82%、69.84%)及超声造影(56.25%、64.56%、66.67%),差异有统计学意义(P<0.05)。超声弹性成像与超声造影的ROC曲线下面积之差为0.033,两种检验方法的诊断价值比较差异无统计学意义,超声弹性成像与联合检测ROC曲线下面积之差为0.304,两种检验方法的诊断价值比较差异有统计学意义(P<0.05),超声造影与联合检测ROC曲线下面积之差为0.271,两种检验方法的诊断价值比较差异有统计学意义(P<0.05),且联合检测的诊断价值高于超声弹性成像、超声造影单独检测。结论超声弹性成像与超声造影均具有一定的诊断价值,但两者联合诊断效果更佳,灵敏度、准确度及特异度均较高,能为乳腺良恶性肿瘤提供可靠的鉴别诊断依据。
Objective To investigate the clinical value of contrast-enhanced ultrasound combined with ultrasound elastography for the differential diagnosis of benign and malignant breast masses.Methods A total of 79 patients with breast masses diagnosed in our hospital from February 2020 to February 2021 were selected as the study subjects,and all patients underwent ultrasound elastography and contrast-enhanced ultrasound examination,and the diagnostic results of ultrasound elastography,contrast-enhanced ultrasound and combined examination were compared based on the results of pathological examination.Results The sensitivity,accuracy and specificity of ultrasound elastography combined with contrast-enhanced ultrasound(93.75%,92.41%and 92.06%)were higher than those of ultrasound elastography(50.00%,68.82%,69.84%)and contrast-enhanced ultrasound(56.25%,64.56%and 66.67%),the difference was statistically significant(P<0.05).The difference of area under ROC curve between ultrasound elasticography and contrast-enhanced ultrasound was 0.033,the difference of area under ROC curve between ultrasonic elastic imaging and combined detection was 0.304,there was significant difference of the diagnostic value of the two methods(P<0.05),the difference of area under ROC curve between contrast-enhanced ultrasound and combined detection was 0.271,there was significant difference of the comparative diagnostic value between the two methods(P<0.05),and the diagnostic value of the combined detection was higher than that of ultrasound elastography and contrast-enhanced ultrasound alone.Conclusion Both ultrasound elastography and contrast-enhanced ultrasound have certain diagnostic value,but their combined diagnosis effect is better,with higher sensitivity,accuracy and specificity,which can provide reliable basis for the differential diagnosis of benign and malignant breast tumors.
