瑞芬太尼联合丙泊酚辅以右美托咪定对肿瘤手术全身麻醉的效果分析

Effect analysis of remifentanil combined with propofol and dexmedetomidine in general anesthesia for tumor surgery

目的探讨瑞芬太尼联合丙泊酚辅以右美托咪定对肿瘤手术全身麻醉的效果。方法选取2020年1月至2021年4月本院收治的80例肿瘤患者作为研究对象,均接受手术治疗,采用随机抽签法分为对照组和研究组,各40例。对照组采用瑞芬太尼联合丙泊酚麻醉,研究组在对照组基础上给予右美托咪定麻醉,比较两组生命体征(平均动脉压、心率)、麻醉效果(拔管时间、睁眼时间)及术后指标[视觉模拟评分法(VAS)评分、镇静评分Ramsay]。结果T0时,两组平均动脉压、心率比较差异无统计学意义;T1、T2、T3、T4时,研究组平均动脉压、心率均低于对照组,差异有统计学意义(P<0.05)。两组拔管时间比较差异无统计学意义;研究组睁眼时间短于对照组,差异有统计学意义(P<0.05)。术后1、4 h,研究组VAS评分均低于对照组,Ramsay评分均高于对照组,差异有统计学意义(P<0.05)。结论在瑞芬太尼复合丙泊酚基础上辅以右美托咪定进行全身麻醉有利于稳定肿瘤患者平均动脉压及心率,缩短睁眼时间,缓解术后疼痛感,提高镇静效果,值得临床推广应用。

Objective To explore the effect of remifentanil combined with propofol and dexmedetomidine in general anesthesia for tumor surgery.Methods 80 tumor patients admitted from January 2020 to April 2021 were selected as study subjects for surgical treatment,and they were divided into control group and study group,with 40 cases in each group.The control group used remifentanil and propofol anesthesia,and the study group gave dexmedetomidine anesthesia based on the control group,the vital signs(mean arterial pressure,heart rate),anesthesia effect(extubation time,eye opening time),and postoperative indicators(visual analogue score[VAS]score,sedation score Ramsay)between two groups were compared.Results At T0,there was no significant difference of the mean arterial pressure and heart rate between the two groups,and at T1,T2,T3 and T4.the mean arterial pressure and heart rate of study group were lower than those of the control groupnd,the difference was statistically significant(P<0.05).There was no significant difference in the extubation time between the two groups,and the eye-opening time was of study group was shorter than that of the control group(P<0.05).At 1 and 4 h after surgery,the VAS score of study group was lower than the control group,the Ramsay score was higher than the control group,and the difference was statistically significant(P<0.05).Conclusion General anesthesia with remifentanil compound propofol supplemented with dexmedetomidine is beneficial to stabilize the arterial pressure and heart rate of tumor patients,shorten the eye opening time,relieve postoperative pain,and improve the sedative effect,which is worthy of clinical application.
