狼疮性肾炎患者肾功能持续恶化的影响因素分析

Analysis of factors influencing renal function deterioration in patients with lupus nephritis

目的分析狼疮性肾炎患者肾功能持续恶化的影响因素。方法选取2019年3月至2020年10月于本院就诊的80例狼疮性肾炎患者(肾功能恶化)作为实验组,另选取同期收治的80例狼疮性肾炎患者(肾功能未恶化)作为对照组,比较两组临床资料,总结肾功能持续恶化的影响因素。结果单因素分析结果显示,狼疮性肾炎患者肾功能持续恶化与感染、低蛋白血症、狼疮活动、高三酰甘油血症、高尿酸血症、24 h尿蛋白定量有关(P<0.05),与性别、年龄、重度贫血无关;多因素分析结果显示,肾功能持续恶化的影响因素包括合并感染、低蛋白血症、高三酰甘油血症、狼疮活动、高尿酸血症、24 h尿蛋白定量≥3.5 g(P<0.05)。结论狼疮性肾炎患者肾功能持续恶化的影响因素多种多样,临床上应综合分析患者的实际病情,给予相应的干预方案,改善患者临床症状,促进身体的早日康复。

Objective To analyze the influencing factors of renal function deterioration in patients with lupus nephritis.Methods A total of 80 patients with lupus nephritis(renal function deterioration)admitted to our hospital from March 2019 to October 2020 were selected as the experimental group,and another 80 patients with lupus nephritis(renal function deterioration)during the same period were selected as the control group.The clinical data of the two groups were compared,and the influencing factors of the continuous deterioration of renal function were analyzed.Results Univariate analysis showed that the continuous deterioration of renal function in patients with lupus nephritis was correlated with infection,hypoproteinemia,lupus activity,hyperacylglyceremia,hyperuricemia and 24 h urinary protein quantification(P<0.05),but not regardless with gender,age or severe anemia.Multivariate analysis showed that the factors affecting renal function deterioration included co-infection,hypoproteinemia,hyperacylglyceremia,lupus activity,hyperuricemia,24 h urinary protein quantity≥3.5 g(P<0.05).Conclusion There are various factors influencing the deterioration of renal function in patients with lupus nephritis.It is necessary to comprehensively analyze the actual condition of patients and give corresponding intervention programs to improve the clinical symptoms of patients and promote the early recovery of the body.
