甲肝乙肝感染性疾病行免疫学检验的临床价值研究

Study on the clinical value of immunological test for hepatitis A,hepatitis B and infectious diseases

目的探讨对甲肝乙肝感染性疾病行免疫学检验的临床价值。方法选取2020年1月至2021年6月本院收治的120例疑似甲肝乙肝患者作为研究对象,根据疾病类型分组,甲肝43例(甲肝组),乙肝41例(乙肝组),健康人36例(健康组),3组均进行免疫学检验和常规检验,比较两种方法检测准确率,比较3组肝功能[白蛋白(ALB),总蛋白(TP),谷氨酰转肽酶(GGT),天冬氨酸转氨酶(AST),谷丙转氨酶(ALT),碱性磷酸酶(ALP),总胆红素(TBil),直接胆红素(DBil)]水平,血清免疫球蛋白[免疫球蛋白M(IgM),免疫球蛋白G(IgG),免疫球蛋白A(IgA)]和补体C3/C4水平。结果免疫学检验检测准确率高于常规检测(P<0.05);甲肝组与乙肝组ALB、TP、GGT、AST、ALT、ALP、TBil、DBil水平比较差异无统计学意义,乙肝组和甲肝组ALB、TP低于健康组,GGT、AST、ALT、ALP、TBil、DBil水平高于健康组(P<0.05);甲肝组IgM、IgG、IgA水平高于乙肝组和健康组,补体C3、C4水平低于乙肝组和健康组(P<0.05),乙肝组IgM、IgG、IgA水平高于健康组,补体C3、C4水平低于健康组(P<0.05)。结论对甲肝乙肝感染性疾病行免疫学检验,诊断准确率较高,具有临床诊断价值。

Objective To explore the clinical value of immunological test for hepatitis A,hepatitis B and infectious diseases.Methods 120 patients with suspected hepatitis A and hepatitis B admitted to our hospital from January 2020 to June 2021 were selected as the research subjects,they were divided into three groups according to the disease type,43 patients with hepatitis A(hepatitis A group),41 patients with hepatitis B(hepatitis B group),and 36 cases healthy people(healthy group).Immunological tests and routine tests were performed in all three groups,and the accuracy of the two methods was compared,liver function(albumin[ALB],total protein[TP],glutamyltransferase[GGT],aspartate aminotransferase[AST],alanine aminotransferase[ALT],alkaline phosphatase[ALP],total bilirubin[TBil],direct bilirubin[DBil])levels,serum immunoglobulin(immunoglobulin M[IgM],immunoglobulin G[IgG],immunoglobulin A[IgA])and complement C3/C4 levels were analyzed in the three groups.Results The accuracy of immunological test was higher than that of routine test(P<0.05).There was no significant difference in the levels of ALB,TP,GGT,AST,ALT,ALP,TBil and DBil between hepatitis A group and hepatitis B group.The levels of ALB and TP in hepatitis B group and hepatitis A group were lower than those in healthy group,while the levels of GGT,AST,ALT,ALP,TBil and DBil in hepatitis A group were higher than those in healthy group(P<0.05).The levels of IgM,IgG and IgA in hepatitis A group were higher than those in hepatitis B group and healthy group,the levels of complement C3 and C4 were lower than those in hepatitis B group and healthy group(P<0.05),the levels of IgM,IgG and IgA in hepatitis B group were higher than those in healthy group,and the levels of complement C3 and C4 were lower than those in healthy group(P<0.05).Conclusion Immunological test for hepatitis A,hepatitis B and infectious diseases has high diagnostic accuracy and clinical diagnostic value.
