MRI联合CT诊断类风湿关节炎患者骨髓水肿和骨侵蚀情况及预后相关性

摘要 目的MRI联合CT诊断类风湿关节炎(RA)患者骨髓水肿和骨侵蚀情况及预后相关性。方法选取2016年1月至2020年12月于本院就诊的68例类风湿关节炎患者作为研究对象,患者均实施CT诊断联合MRI诊断、单一诊断,比较诊断结果。结果68例患者经诊断后发现,骨髓水肿在181块骨中检出,骨侵蚀在120块骨中检出,骨髓水肿类型主要是月骨,骨侵蚀主要为腕骨,其中关节受累有32例,占比47.06%(32/68),MRI联合CT诊断方式的检出率为100.00%(68/68)明显高于单一诊断(CT)的70.59%(48/68),差异有统计学意义(χ^(2)=12.173,P<0.05)。结论CT联合MRI诊断类风湿关节炎患者骨髓水肿和骨侵蚀效果显著,具有较高的检出率,能清晰观察患者的具体病变,值得临床推广应用。 Objective To investigate the correlation between MRI and CT in diagnosis of bone marrow edema and bone erosion in patients with rheumatoid arthritis(RA)and prognosis.Methods 68 patients with rheumatoid arthritis who were treated in our hospital from January 2016 to December 2020 were selected as the study subjects,CT diagnosis combined with MRI diagnosis and single diagnosis were performed of patients,and the diagnosis results were compared.Results After diagnosis,bone marrow edema was detected in 181 bones and bone erosion was detected in 120 bones in 68 patients.The type of bone marrow edema was mainly lunate bone,and bone erosion was mainly carpal bone.Among them,there were 32 cases with joint involvement,accounting for 47.06%(32/68).The detection rate of MRI combined with CT was 100.00%(68/68),which was significantly higher than 70.59%(48/68)of single diagnosis(CT),the difference was statistically significant(χ^(2)=12.173,P<0.05).Conclusion CT combined with MRI is effective in the diagnosis of bone marrow edema and bone erosion in patients with rheumatoid arthritis,and has a high detection rate,it can clearly observe the specific lesions of patients,which is worthy of clinical promotion and application.
