超声引导下穿刺活检和术中快速冰冻组织活检应用于乳腺肿块性状诊断中的价值对比

Comparison of the value of ultrasound-guidance needle biopsy and intraoperative rapid frozen tissue biopsy in the diagnosis of breast mass traits

目的探究超声引导下穿刺活检和术中快速冰冻组织活检应用于乳腺肿块性状诊断中的价值。方法选取2019年12月至2020年8月湖南省中医药研究院附属医院病理科诊断的132例乳腺肿块患者作为研究对象,均采用超声引导下穿刺活检、术中快速冰冻组织活检,以术后石蜡病理学检查结果作为金标准,比较上述两种方法检测乳腺肿块性状的价值。结果132例乳腺肿块患者,经术后石蜡病理学检查确诊112例良性肿瘤,20例恶性肿瘤,其中术中快速冰冻组织检测115例良性肿瘤,17例恶性肿瘤;超声引导下穿刺活检检测113例良性肿瘤,19例恶性肿瘤。术中快速冰冻组织检测乳腺肿块的特异度(97.32%)、灵敏度(70.00%)、阳性预测值(82.35%)及诊断准确率(93.18%)均高于超声引导下穿刺活检的89.29%、35.00%、36.84%、81.06%,差异有统计学意义(P<0.05)。两组阴性预测值比较差异无统计学意义。结论相较于超声引导下穿刺活检,术中快速冰冻组织活检可提高对乳腺肿块的检测诊断,值得临床推广应用。

Objective To investigate the value of ultrasound-guidance needle biopsy and intraoperative rapid frozen tissue biopsy in the diagnosis of breast mass traits.Methods A total of 132 patients with breast masses diagnosed by the Pathology Department of the Affiliated Hospital of Hunan Academy of Traditional Chinese Medicine from December 2019 to August 2020 were selected as the research subjects,all patients underwent ultrasound-guidance needle biopsy and intraoperative rapid frozen tissue biopsy.The postoperative paraffin pathological examination results were used as the gold standard,the value of the above two methods in detecting the characteristics of breast masses were compared.Results Among the 132 patients with breast masses,112 cases of benign tumors and 20 cases of malignant tumors were confirmed by postoperative paraffin pathological examination,including 115 benign tumors and 17 malignant tumors detected by intraoperative rapid frozen tissue.Ultrasound-guidance needle biopsy detected 113 benign tumors and 19 malignant tumors.The specificity(97.32%),sensitivity(70.00%),positive predictive value(82.35%)and diagnostic accuracy(93.18%)of breast mass detection by rapid freezing tissue during surgery were higher than 89.29%,35.00%,36.84%and 81.06%of breast mass detection under ultrasound-guided needle biopsy,and the differences were statistically significant(P<0.05).There was no significant difference in the negative predictive values between the two groups.Conclusion Compared with ultrasound-guidance needle biopsy,intraoperative rapid frozen tissue biopsy can improve the detection and diagnosis of breast masses,which is worthy of clinical application.
