纳布啡复合全身麻醉在眼科日间手术患儿中的应用价值

Application of nabuphine combined general anesthesia in children with ophthalmic day surgery

目的探究纳布啡复合全身麻醉在眼科日间手术患儿中的应用价值。方法回顾性分析2019年7月至2020年6月本院收治的87例行眼科日间手术患儿的临床资料,按照治疗方式不同分为研究组(n=46)、对照组(n=41),两组均使用潮气量法以7%七氟烷吸入诱导后置入喉罩,手术过程中保持自主呼吸,静脉开放后,研究组给予0.2 mg/kg纳布啡麻醉,对照组给予相应剂量0.9%氯化钠溶液麻醉,比较两组诱导后给药前(T_(1))、给药10 min后手术开始时(T_(2))、手术结束时(T_(3))的心率(HR)、分钟通气量(MV)及苏醒时间、清醒时间、不良反应发生情况。结果两组HR、MV组间、时间、交互比较差异有统计学意义;两组T_(1)、T_(3)时的HR、MV比较差异无统计学意义;T_(2)时研究组HR、MV均明显低于对照组,差异有统计学意义(P<0.05)。研究组苏醒时间、清醒时间均显著短于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论纳布啡复合全身麻醉应用于眼科日间术患儿疗效较好,可有效改善患者心率、分钟通气量,促进患者康复,减少不良反应发生,值得临床推广应用。

Objective To investigate the value application of nabuphine combined general anesthesia in children undergoing ophthalmic day surgery.Methods The clinical data of 87 children undergoing ophthalmic day surgery admitted to our hospital from July 2019 to June 2020 were retrospectively analyzed,and they were divided into research group(n=46)and control group(n=41)according to their treatment methods,both groups were induced by 7%sevoflurane inhalation by tidal volume method and inserted into the laryngeal mask,spontaneous breathing was maintained during the operation,and after the vein was opened,the study group was given nalbuphine 0.2 mg/kg anesthesia,and the control group was given the corresponding dose of normal saline anesthesia.The heart rate(HR),minute ventilation(MV),wake time,wakefulness time,and adverse reactions of the two groups before administration(T_(1))and 10 minutes after administration were compared between two groups.Results There were significant differences of HR and MV two groups of inter-group and interaction and time(P<0.05);there was no significant difference in HR and MV between the two groups at T_(1) and T_(3),and the HR and MV in the study group at T_(2)were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).The awakening time and awakening time of the study group were significantly shorter than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Nabuphine combined with general anesthesia has a good effect on ophthalmic day surgery in children,which can effectively improve the patient's heart rate,minute ventilation,promote the recovery of patients,and reduce the occurrence of adverse reactions,which is worthy of clinical promotion and application.
