非体外循环下心脏搭桥术对冠心病患者术后出血量及康复进程的影响

Effect of off-pump heart bypass surgery on postoperative blood loss and rehabilitation of patients with coronary heart disease

目的探究非体外循环下心脏搭桥术对冠心病患者术后出血量及康复进程的影响。方法回顾性分析2017年6月至2020年6月本院收治的70例冠心病患者临床资料,根据治疗方法不同分为对照组和观察组,每组35例。对照组行体外循环搭桥术,观察组行非体外循环搭桥术。比较两组手术情况、术后并发症发生情况及手术前后36项简明健康调查量表(SF-36)评分。结果观察组ICU入住时间、手术时间、呼吸机辅助时间及住院时间均短于对照组,术后出血量少于对照组,差异有统计学意义(P<0.05)。观察组术后并发症发生率为34.29%,对照组的82.86%,差异有统计学意义(P<0.05)。术后,观察组SF-36评分高于对照组,差异有统计学意义(P<0.05)。结论非体外循环搭桥手术能缩短手术、ICU入住及呼吸机辅助时间,减少术后出血量,降低术后并发症发生风险,有利于提高患者的生命质量,值得临床推广应用。

Objective To investigate the effect of off-pump heart bypass surgery on postoperative blood loss and rehabilitation process in patients with coronary heart disease.Methods The clinical data of 70 patients with coronary heart disease admitted to our hospital from June 2017 to June 2020 were analyzed retrospectively,they were divided into control group and observation group according to different treatment methods,with 35 patients in each group.The control group received cardiopulmonary bypass bypass,while the observation group received off-pump heart bypass surgery.The operation condition,postoperative complications and the score of 36-item short form health survey(SF-36)before and after operation were compared between the two groups.Results The ICU stay time,operation time,ventilator assistance time and hospital stay in the observation group were shorter than those in the control group,and the postoperative blood loss was less than that in the control group,the difference was statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 34.29%,which was lower than 82.86%in the control group,and the difference was statistically significant(P<0.05).After surgery,the SF-36 score of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Off-pump bypass surgery can shorten the time of operation,ICU stay and ventilator support,reduce postoperative blood loss,and the risk of postoperative complications,and help to improve the quality of life of patients,which is worthy of clinical application.
