支气管镜肺泡灌洗联合甲泼尼龙治疗支原体肺炎的效果观察

Effect of bronchoscopic alveolar lavage combined with methylprednisolone in the treatment of mycoplasma pneumonia

目的探讨支气管镜肺泡灌洗联合甲泼尼龙治疗支原体肺炎的效果。方法选取2018年1月至2020年1月本院收治的186例支原体肺炎患儿作为研究对象,随机分为对照组与观察组,各93例。所有患儿均给予常规治疗,对照组予以甲泼尼龙治疗,观察组予以支气管镜肺泡灌洗联合甲泼尼龙治疗。比较两组疗效评价指标、临床疗效、实验室指标[C反应蛋白(CRP)、铁蛋白(SF)、降钙素原(PCT)、D-二聚体(D-D)]。结果观察组总热程、咳嗽消失时间、肺部体征消失时间、胸片恢复正常时间均短于对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率为97.85%,高于对照组的88.17%,差异有统计学意义(P<0.05)。治疗后,两组CRP、SF、PCT、D-D水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论支气管镜肺泡灌洗联合甲泼尼龙治疗支原体肺炎疗效显著,可明显缩短患儿恢复时间,减轻炎症反应,值得临床推广应用。

Objective To investigate the effect of bronchoscopic alveolar lavage combined with methylprednisolone in the treatment of mycoplasma pneumonia.Methods A total of 186 children witc mycoplasma pneumonia admitted to our hospital from January 2019 to January 2021 were selected as the research subjects and they were randomly divided into control group and observation group,with 93 cases in each group.All children were given routine treatment,the control group was treated with methylprednisolone,and the observation group was treated with bronchoalveolar lavage combined with methylprednisolone.The efficacy evaluation indexes,clinical efficacy,laboratory indexes(C-reactive protein[CRP],ferritin[SF],procalcitonin[PCT],D-dimer[D-D])between the two groups were compared.Results The total heat course,cough disappearance time,lung signs disappearance time and chest radiograph recovery time of the observation group were shorter than those of the control group,the difference was statistically significant(P<0.05).The total effective rate of the observation group was 97.85%,which was higher than 88.17% of the control group,and the difference was statistically significant(P<0.05).After treatment,CRP,SF,PCT and D-D levels of the two groups were lower than before treatment,and observation group was lower than control group,the differences were statistically significant(P<0.05).Conclusion Bronchoalveolar lavage combined with methylprednisolone is effective in the treatment of the mycoplasma pneumonia,it can shorten the recovery time of the body and reduce the inflammatory response,which is worthy of clinical application.
