剖宫产术联合子宫肌瘤剔除术应用改良式VLOC单向免打结可吸收缝合线棒球缝合法的临床研究

Clinical efficacy of caesarean section combined with myomectomy using modified VLOC one-way knot-free absorbable suture baseball suture

目的探讨剖宫产术联合子宫肌瘤剔除术应用改良式VLOC单向免打结可吸收缝合线棒球缝合法的临床疗效。方法选取2017年12月至2020年12月本院收治的100例妊娠合并子宫肌瘤剖宫产患者作为研究对象,随机分为对照组和观察组,每组50例。对照组采用剖宫产术联合子宫肌瘤剔除术应用传统缝合法,观察组采用剖宫产术联合子宫肌瘤剔除术应用改良式VLOC单向免打结可吸收缝合线棒球缝合法,比较两组术中指标(手术时间、术中出血量)、术后指标(血红蛋白下降值、术后出血量、肛门排气时间、月经恢复时间、恶露干净时间、住院时间)及并发症发生情况。结果观察组手术时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05)。两组肛门排气时间、月经恢复时间、恶露干净时间、住院时间比较差异无统计学意义;观察组血红蛋白下降值低于对照组,术后出血量少于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为4.00%,明显低于对照组的18.00%,差异有统计学意义(P<0.05)。结论剖宫产术联合子宫肌瘤剔除术应用改良式VLOC单向免打结可吸收缝合线棒球缝合法效果显著,可缩短手术时间,减少术中、术后出血量,且安全性高,值得临床推广应用。

Objective To investigate the clinical efficacy of caesarean section combined with myomectomy using modified VLOC one-way knot-free absorbable suture baseball suture.Methods 100 pregnant women with hysteromyoma undergoing cesarean section admitted to our hospital from December 2017 to December 2020 were selected as research subjects,and they were randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with cesarean section combined with myomectomy using traditional suture method,the observation group was treated with cesarean section combined with myomectomy using modified VLOC one-way knot-free absorbable suture baseball suture method,the intraoperative indicators(operation time,intraoperative blood loss),postoperative indicators(hemoglobin decline value,postoperative blood loss,anal exhaust time,menstrual recovery time,lotion clean time,hospital stay)and complications were compared between the two groups.Results The operation time of the observation group was shorter than that of the control group,and the amount of intraoperative bleeding was less than that of the control group,the difference was statistically significant(P<0.05).There was no significant difference in anus exhaust time,menstruation recovery time,lochia clean time and hospitalization time between the two groups;the decrease of hemoglobin in the observation group was lower than that in the control group,and the amount of postoperative bleeding was less than that in the control group,the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 4.00%,which was significantly lower than 18.00% in the control group,and the difference was statistically significant(P<0.05).Conclusion The application of modified VLOC one-way knot-free absorbable suture baseball suture in cesarean section combined with hysteromyomectomy has significant effect,can shorten the operation time,reduce intraoperative and postoperative bleeding,and has high safety,which is worthy of clinical promotion.
