血清血清甲胎蛋白、糖类抗原19-9、癌胚抗原及铁蛋白联合检测在结直肠癌肝转移中的诊断价值

The combined detection value of serum alpha-fetoprotein,carbohydrate antigen 19-9,carcinoembryonic antigen,and ferritin in liver metastasis of colorectal cancer

目的分析血清甲胎蛋白(AFP)、糖类抗原19-9(CA19-9)、癌胚抗原(CEA)及铁蛋白(Fer)联合检测在结直肠癌肝转移中的诊断价值。方法回顾性分析2016年5月至2020年10月本院收治的104例结直肠癌患者的临床资料,根据是否有肝转移分为结直肠癌肝转移组与结直肠癌肝未转移组,每组52例。比较两组血清AFP、CA19-9、CEA及Fer水平及其阳性率,AFP、CA19-9、CEA及Fer单独及联合检测对结直肠癌肝转移的诊断价值。结果结直肠癌肝转移组血清CA19-9、CEA及Fer水平均明显高于结直肠癌肝未转移组,差异有统计学意义(P<0.05),两组AFP水平比较差异无统计学意义。结直肠癌肝转移组血清AFP、Fer阳性率均高于结直肠癌肝未转移组,差异有统计学意义(P<0.05)。联合诊断结直肠癌诊断的灵敏度、准确度均高于血清AFP、CA19-9、CEA、Fer单独诊断,差异有统计学意义(P<0.05)。结论血清AFP、CA19-9、CEA及Fer联合检测可提高结直肠癌肝转移诊断的特异度及准确度,临床诊断价值较高。

Objective To analyze the diagnostic value of serum alpha-fetoprotein(AFP),carbohydrate antigen 19-9(CA19-9),carcinoembryonic antigen(CEA)and ferritin(Fer)in liver metastasis of colorectal cancer.Methods The clinical data of 52 patients with colorectal cancer liver metastasis admitted to our hospital from May 2016 to October 2020 were retrospectively analyzed and included in the colorectal cancer liver metastasis group,and 52 patients with colorectal cancer liver metastasis were selected as the colorectal cancer liver metastasis group.The levels and positive rates of serum AFP,CA19-9,CEA and Fer were compared between the two groups,the diagnostic value of AFP,CA19-9,CEA and Fer alone and in combination in colorectal cancer liver metastasis.Results The serum CA19-9,CEA and Fer levels in the colorectal cancer liver metastasis group were significantly higher than those in the colorectal cancer liver metastasis group,and the difference was statistically significant(P<0.05),and there was no significant difference in AFP level between the two groups The serum AFP and Fer positive rates in the colorectal cancer liver metastasis group were higher than those in the colorectal cancer liver metastasis group and the difference was statistically significant(P<0.05).The sensitivity and accuracy of combined diagnosis of colorectal cancer were higher than those of AFP,CA19-9,CEA and Fer alone,and the differences were statistically significant(P<0.05).Conclusion The combined detection of serum AFP,CA19-9,CEA and Fer can improve the specificity and accuracy of the diagnosis of colorectal cancer liver metastasis,and has high clinical diagnostic value.
