术前呼吸训练联合快速康复外科护理对肺叶切除术患者肺功能恢复的影响

Effect of Preoperative Respiratory Training Combined with Rapid Rehabilitation Surgical Nursing on Pulmonary Function Recovery in Patients Undergoing Lobectomy

目的研究术前呼吸训练联合快速康复外科护理对肺叶切除术患者肺功能恢复的影响。方法选取2022年1月-6月在我院择期行肺叶切除术的84例患者为研究对象,采用随机数字表法分为对照组和观察组,各42例。对照组采用常规护理,观察组在对照组基础上采用术前呼吸训练联合快速康复外科护理,比较两组临床手术指标、肺功能指标[动脉血氧分压(PaO_(2))、用力肺活量(FVC)、1 s用力呼气量(FEV_(1))及最大深吸气量(IC)]、并发症发生率以及生活质量评分。结果观察组胃肠功能恢复时间、下床活动时间、住院时间均短于对照组(P<0.05);两组护理后PaO_(2)、FVC、FEV_(1)、IC均大于术前,且观察组均大于对照组(P<0.05);观察组并发症发生率为7.14%,低于对照组的19.05%(P<0.05);观察组患者生活质量各维度评分均高于对照组(P<0.05)。结论术前呼吸训练联合快速康复外科护理可促进肺叶切除术患者术后缩短胃肠功能恢复、下床活动以及住院时间,促进肺功能恢复,预防并发症,提高患者生活质量水平,具有理想的应用效果。

Objective To study the effect of preoperative respiratory training combined with rapid rehabilitation surgical nursing on pulmonary function recovery in patients undergoing lobectomy.Methods A total of 84 patients who underwent elective lobectomy in our hospital from January to June 2022 were selected as the research objects.They were divided into control group and observation group by random number table method,with 42 patients in each group.The control group was given routine nursing,and the observation group was given preoperative respiratory training combined with rapid rehabilitation surgical nursing on the basis of the control group.The clinical operation indexes,pulmonary function indexes[arterial partial pressure of oxygen(PaO_(2)),forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1))and maximum inspiratory capacity(IC)],complication rate and quality of life score were compared between the two groups.Results The recovery time of gastrointestinal function,the time of getting out of bed and the time of hospitalization in the observation group were shorter than those in the control group(P<0.05).After nursing,PaO_(2),FVC,FEV_(1) and IC in the two groups were higher than those before operation,and those in the observation group were higher than the control group(P<0.05).The incidence of complications in the observation group was 7.14%,which was lower than 19.05%in the control group(P<0.05).The scores of all dimensions of quality of life in the observation group were higher than those in the control group(P<0.05).Conclusion Preoperative respiratory training combined with rapid rehabilitation surgical nursing can promote the recovery of gastrointestinal function,ambulation and hospitalization time,promote the recovery of lung function,prevent complications and improve the quality of life of patients after lobectomy,which has an ideal application effect.
