盆底神经肌肉刺激对产后盆底功能障碍与盆底肌纤维收缩的影响

Effects of pelvic floor neuromuscular stimulation on postpartum pelvic floor dysfunction and pelvic floor muscle fiber contraction

摘要 目的分析盆底神经肌肉刺激对产后盆底功能障碍与盆底肌纤维收缩的影响。方法回顾性分析2016年12月至2018年6月于本院分娩的140名产妇的临床资料,将采用Kegel运动干预的70名产妇纳入参照组,采用盆底神经肌肉刺激干预的70名产妇纳入观察组。比较两组盆底功能障碍发生率、阴道肌力、盆底肌纤维收缩时间、盆底肌纤维持续收缩压力、女性性功能指数(FSFI)和盆底功能影响问卷简表-7(PFIQ-7)评分。结果观察组尿失禁、盆腔痛、性欲下降发生率均低于参照组,差异有统计学意义(P<0.05)。干预2个月后,观察组阴道肌力大于参照组,盆底肌纤维收缩时间长于参照组,差异有统计学意义(P<0.05)。干预2个月后,观察组盆底肌Ⅰ类、Ⅱ类肌纤维持续收缩压均大于参照组,差异有统计学意义(P<0.05)。干预2个月后,观察组FSFI、PFIQ-7评分均高于参照组,差异有统计学意义(P<0.05)。结论分娩产妇采用产后盆底神经肌肉刺激干预效果显著,可有效减少产后盆底功能障碍的发生,促进盆腔功能恢复,提升产妇生命质量。 Objective To analyze the effect of pelvic floor neuromuscular stimulation on postpartum pelvic floor dysfunction and pelvic floor muscle fiber contraction.Methods The clinical data of 140 puerpera who delivered in our hospital from December 2016 to June 2018 were retrospectively analyzed,70 puerpera who received Kegel exercise intervention were included in the reference group,and 70 puerpera who received pelvic floor neuromuscular stimulation intervention were included in the observation group.The incidence of pelvic floor dysfunction,vaginal muscle strength,contraction time of pelvic floor muscle fiber,continuous contraction systolic pressure on pelvic floor muscle fiber,female sexual function index(FSFI)and pelvic floor impact questionnaire short form-7(PFIQ-7)scores were compared between the two groups.Results The incidence of urinary incontinence,pelvic pain and decreased libido in the observation group were lower than those in the reference group,and the difference was statistically significant(P<0.05).After 2 months of intervention,the vaginal muscle strength of the observation group was greater than that of the reference group,and the contraction time of pelvic floor muscle fiber was longer than that of the reference group,and the difference was statistically significant(P<0.05).After 2 months of intervention,the continuous contraction systolic pressure of typeⅠand typeⅡpelvic floor muscle fiber in the observation group were higher than those in the reference group,and the differences were statistically significant(P<0.05).After 2 months of intervention,the FSFI and PFIQ-7 scores of the observation group were higher than those of the reference group,and the differences were statistically significant(P<0.05).Conclusion Postpartum pelvic floor neuromuscular stimulation has significant effects on puerpera,which can effectively reduce the occurrence of postpartum pelvic floor dysfunction,promote pelvic function recovery,and improve the quality of life of puerpera.
