彩色多普勒超声诊断左心室肥大合并左心衰竭的临床价值

Clinical value of color Doppler ultrasound in the diagnosis of left ventricular hypertrophy complicated with left heart failure

目的探究彩色多普勒超声诊断左心室肥大合并左心衰竭的临床价值。方法选取2019年4月至2020年12月本院收治的50例左心室肥大合并左心衰竭患者纳入实验组,另选取同期于本院进行体检的50名健康人纳入对照组,比较两组彩色多普勒超声检查结果及不同美国纽约心脏病协会(NYHA)分级患者的彩色多普勒超声检查结果。结果实验组心肌作功指数(Tei指数)高于对照组,室间隔厚度(IVST)、左心室后壁厚度(LVPWT)、均厚于对照组,左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)、左心房内径(LAD)均长于对照组,实验组二尖瓣舒张早期最大血流速度/舒张晚期最大血流速度(E/A)值、左室射血分数(LVEF)、左室短轴缩短率(LVFS)均低于对照组,差异有统计学意义(P<0.05)。不同NYHA分级患者间Tei指数、E/A比值、LVEF、IVST、LVPWT、LVFS、LVESD、LVEDD、LAD比较差异有统计学意义(P<0.05);不同NYHA分级患者间E/A比值、LVEF、LVFS比较差异无统计学意义。结论彩色多普勒心脏超声能根据Tei指数、E/A比值、LVEF、IVST等指标精准评估心功能,对准确诊断左心室肥大合并左心衰竭具有重要意义。

Objective To investigate the clinical value of color Doppler ultrasound in the diagnosis of left ventricular hypertrophy complicated with left heart failure.Methods 50 patients with left ventricular hypertrophy complicated with left heart failure who were treated in our hospital from April 2019 to December 2020 were selected and included in the experimental group,and 50 healthy people who underwent physical examination in our hospital during the same period were selected and included in the control group.Doppler ultrasound results were caopared betweem the two group and color Doppler ultrasound results of patients with different New York Heart Association(NYHA)classifications were compared.Results The myocardial work index(Tei index),interventricular septal thickness(IVST),left ventricular posterior wall thickness(LVPWT),left ventricular endsystolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD),and left atrial diameter(LAD)in the experimental group were higher than those in the control group,the ratio of the blood flow velocitv of mitralannulus during early diastole/the blood flow velocity of mitralannulus during atrial contraction(E/A),left ventricular ejection fraction(LVEF),and left ventricular fractional shortening(LVFS)were all lower than those in the control group,and the difference was statistical significance(P<0.05).There were significant differences in Tei index,E/A ratio,LVEF,IVST,LVPWT,LVFS,LVESD,LVEDD,LAD among patients with different NYHA grades(P<0.05);there was no significant difference in E/A ratio,LVEF and LVFS among patients with different NYHA grades.Conclusion Color Doppler echocardiography can accurately assess cardiac function according to Tei index,E/A ratio,LVEF,IVST and other indicators,which is of great significance for the accurate diagnosis of left ventricular hypertrophy complicated with left heart failure.
