超声引导下神经阻滞复合全身麻醉在老年髋关节置换术中的应用效果

Application effect of ultrasound-guided nerve block combined with general anesthesia in elderly hip arthroplasty

目的探究超声引导下神经阻滞复合全身麻醉在老年髋关节置换术中的应用效果。方法选取2018年4月至2020年7月于本院接受髋关节置换术治疗的110例老年患者作为研究对象,按照随机数字表法分为实验组和对照组,每组55例。实验组给予超声引导下神经阻滞复合全身麻醉,对照组给予全身麻醉。比较两组术后不同时间视觉模拟评分法(VAS)评分、手术相关指标及不良反应发生情况。结果术后2、4、12 h及术后1、2 d,实验组VAS评分均低于对照组,差异有统计学意义(P<0.05)。实验组下床活动时间、拔管时间、出院时间均短于对照组,差异有统计学意义(P<0.05)。实验组不良反应发生率明显低于对照组,差异有统计学意义(P<0.05)。结论超声引导下神经阻滞复合全身麻醉应用于老年髋关节置换术中效果显著,可缓解患者疼痛,缩短拔管时间及下床活动时间,利于患者早日康复出院,安全性高,值得临床推广应用。

Objective To explore the effect of ultrasound-guided nerve block combined with general anesthesia in elderly hip arthroplasty.Methods 110 elderly patients who underwent hip arthroplasty in our hospital from April 2018 to July 2020 were selected as the research subjects,and they were divided into the experimental group and the control group according to the random number table method,with 55 cases in each group.The experimental group was given ultrasound-guided nerve block combined with general anesthesia,and the control group was given general anesthesia.The visual analog scale(VAS)scores at different time after surgery,operation-related indexes and adverse reactions were compared between the two groups.Results At 2,4,12 h and 1,2 d after operation,the VAS scores of the experimental group were lower than those of the control group,and the differences were statistically significant(P<0.05).The ambulation time,extubation time and discharge time of the experimental group were shorter than those of the control group,and the differences were statistically significant(P<0.05).The incidence of adverse reaction in the experimental group were lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Ultrasoundguided nerve block combined with general anesthesia has significant effect in elderly hip arthroplasty,which can relieve pain,shorten extubation time and ambulation time,facilitate early recovery and discharge of patients,and has high safety,which is worthy of clinical application.
