中西医结合治疗睑板腺功能障碍患者白内障术后干眼症的效果及对泪膜稳定性的影响

Effect of integrated Traditional Chinese andWestern Medicine in the treatment of dry eye after cataract surgery in patients with meibomian gland dysfunction and its effect on tear film stability

目的探讨中西医结合治疗睑板腺功能障碍患者白内障术后干眼症的效果及对泪膜稳定性的影响。方法选取2019年9月至2020年9月本院收治的128例睑板腺功能障碍白内障术后干眼症患者作为研究对象,按照随机双盲法分为对照组与研究组,各64例。对照组采用西医治疗,研究组采用中西医结合治疗,比较两组临床疗效、治疗前后中医症候积分及泪膜稳定性。结果研究组治疗总有效率为90.63%,高于对照组的76.56%,差异有统计学意义(P<0.05)。治疗前,两组双目干涩、灼热感、羞明惧光、赤丝盘虬、视物不清症候积分比较差异无统计学意义;治疗6个月后,两组双目干涩、灼热感、羞明惧光、赤丝盘虬、视物不清症候积分均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05)。两组首次泪膜破裂时间、平均泪膜破裂时间、非侵入式泪河高度时间、组间、交互比较差异有统计学意义(P<0.05)。治疗前,两组首次泪膜破裂时间、平均泪膜破裂时间、非侵入式泪河高度比较差异无统计学意义;治疗6个月后,两组首次泪膜破裂时间、平均泪膜破裂时间均长于治疗前、治疗1 d后、治疗3个月后,非侵入式泪河高度均高于治疗前、治疗1 d后、治疗3个月后,差异有统计学意义(P<0.05);治疗1 d后、治疗3个月后、治疗6个月后,研究组首次泪膜破裂时间、平均泪膜破裂时间均长于对照组,非侵入式泪河高度均高于对照组,差异有统计学意义(P<0.05)。结论中西医结合治疗睑板腺功能障碍白内障术后干眼症的疗效确切,有利于促进患者症状缓解,加强泪膜稳定性。

Objective To investigate the effect of integrated Traditional Chinese and Western Medicine in the treatment of dry eye syndrome after cataract surgery in patients with meibomian gland dysfunction and its effect on tear film stability.Methods A total of 128 patients with dry eye after meibomian gland dysfunction cataract surgery from September 2019 to September 2020 were selected as research subjects,there were divided into control group and research group according to the random double-blind method with 64 cases in each group.The control group was treated withWestern Medicine,and the study group was treated with integrated Traditional Chinese andWestern Medicine,and the clinical efficacy,Traditional Chinese Medicine symptom score and tear film stability before and after treatment were compared between the two groups.Results The total effective rate of treatment in the study group was 90.63%,which was higher than 76.56%in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in the scores of binocular dryness,burning sensation,shame and photophobia,red silk pan,and blurred vision between the two groups;after 6 months of treatment,the scores of binocular dryness,burning sensation,shame and photophobia,red silk discrepancies and blurred vision in the two groups were lower than before treatment,and the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).There were significant differences of first tear film rupture time,mean tear film rupture time,non-invasive tear river height time,between the two groups of inter-group,time and interaction(P<0.05).Before treatment,there were no significant differences in the time of first tear film rupture,mean tear film rupture time,and non-invasive tear river height between the two groups;after 6 months of treatment,the first tear film rupture time and average tear film rupture time of the two groups were longer than those before treatment,1 d after treatment and 3 months after treatment,and the height of non-invasive tear river was higher than that before treatment,and the differences were statistically significant(P<0.05),and 1 d after treatment,3 months after treatment,and 6 months after treatment,the first tear film rupture time and average tear film rupture time in the study group were longer than those in the control group,and the height of non-invasive tear river was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion The efficacy of integrated Traditional Chinese andWestern Medicine in the treatment of dry eye after meibomian gland dysfunction cataract surgery is definite,which is conducive to promoting symptom relief and strengthening the stability of tear film.
