三维适形与3D辅助腔内后装放疗治疗中晚期宫颈癌的临床疗效

Clinical efficacy of 3-dimensional conformal radiation therapy and 3D assisted posterior radiotherapy for middle and advanced cervical cancer

目的探究三维适形与3D辅助腔内后装放疗治疗中晚期宫颈癌的临床疗效。方法选取2019年7月至2020年7月于本院就诊的72例中晚期宫颈癌患者作为研究对象,按照随机分组法分为观察组、对照组,每组36例。对照组采用三维调强放疗联合二维腔内后装治疗,观察组则采用三维调强放疗联合3D辅助腔内后装放疗方式,两组均给予顺铂治疗。比较两组临床疗效和急性不良反应(血管异常、泌尿系损伤如放射性膀胱炎及消化道损伤)发生情况。结果观察组治疗总有效率为97.22%,高于对照组的83.33%,差异有统计学意义(P<0.05);观察组血象异常总发生率为33.33%、泌尿系统损伤总发生率为25.00%、消化道损伤总发生率为22.22%,均低于对照组的58.33%、47.22%、47.22%,差异有统计学意义(P<0.05)。结论三维适形放疗与3D辅助腔内后装放疗治疗中晚期宫颈癌疗效确切,可减少急性不良反应发生情况,安全性较高,促进患者康复,值得临床推广应用。

Objective To investigate the clinical efficacy of 3-dimensional conformal radiation therapy and 3D assisted posterior radiotherapy in the treatment of advanced cervical cancer.Methods A total of 72 patients with advanced cervical cancer who visited our hospital from July 2019 to July 2020 were selected as the research subjects,and they were divided into observation group and control group according to the randomization method,with 36 cases in each group.The control group used the 3-dimensional conformal radiation therapyand combined with two-dimensional intraluminal after-loading therapy,while the observation group used three-dimensional intensity-modulated radiotherapy combined with 3D assisted intraluminal afterloading radiotherapy,and cisplatin therapy was given to both groups.The clinical efficacy and occurrence of acute adverse effects(vascular abnormalities,urinary tract injuries such as radiation cystitis and gastrointestinal injury)were compared between the two groups.Results The total effective rate of treatment in the observation group was 97.22%,which was higher than 83.33%of the control group,and the difference was statistically significant(P<0.05),the total incidence of blood abnormalities in the observation group was 33.33%,the total incidence of urinary system injury was 25.00%,and the total incidence of gastrointestinal injury was 22.22%,which were lower than 58.33%,47.22%and 47.22%in the control group,and the difference was statistically significant(P<0.05).Conclusion 3-dimensional conformal radiation therapy and 3D assisted posterior radiotherapy have definite efficacy in the treatment of intermediate and advanced cervical cancer,can reduce the occurrence of acute adverse reactions,have higher safety,promote the recovery of patients,and are worthy of clinical promotion and application.
