摘要
目的探讨呼出气一氧化氮(FeNO)测定在支气管哮喘诊治中的价值及其与肺功能的关系。方法选取2017年7月至2019年10月本院收治的85例支气管哮喘患者作为观察组,另选取同期于本院体检的85名健康体检者作为对照组,两组均行肺功能、FeNO检测并填写哮喘控制测试(ACT)评分表。观察组根据FeNO检测结果分为FeNO升高组(n=41)和FeNO正常组(n=44),均给予沙美特罗/氟替卡松治疗。比较观察组与对照组肺功能指标[第1秒用力呼气容积(FEV_(1))、FEV_(1)/用力肺活量(FVC)]和FeNO水平,比较FeNO升高组与FeNO正常组哮喘完全控制率和治疗前后FEV_(1)、FEV_(1)/FVC、FeNO水平及ACT评分。结果观察组FeNO水平高于对照组,FEV_(1)、FEV_(1)/FVC均低于对照组,差异有统计学意义(P<0.05)。FeNO升高组哮喘完全控制率高于FeNO正常组,差异有统计学意义(P<0.05)。治疗后,FeNO升高组与FeNO正常组FEV_(1)、FEV_(1)/FVC及ACT评分均高于治疗前,且FeNO升高组高于FeNO正常组,差异有统计学意义(P<0.05)。结论支气管哮喘患者FeNO呈高水平表达,FeNO与肺功能指标检测可为临床诊断和治疗、管理支气管哮喘提供指导,增强支气管哮喘的诊治和临床控制效果,具有较高的应用价值。
Objective To explore the value of exhaled nitric oxide(FeNO)in the diagnosis and treatment of bronchial asthma and its correlation with pulmonary function.Methods 85 patients with asthma treated in our hospital from July 2017 to October 2019 were selected as the observation group,and another 85 healthy physical examiners who underwent physical examination in our hospital at the same time were selected as the control group.Lung function and FeNO were detection,and asthma control test(ACT)scores were filled in the two groups.The patients in the observation group were divided into the elevated FeNO group(n=41)and the normal FeNO group(n=44)according to the FeNO test detection results,all of which were treated with salmeterol/fluticasonet.The pulmonary function indexes[forced expiratory volume in one second(FEV_(1)),FEV_(1)/forced vital capacity(FVC)]and FeNO levels were compared between the observation group and the control group,and the complete control rate of asthma and the asthma control situation,FEV_(1),FEV_(1)/FVC,FeNO levels,and ACT scores before and after treatment were compared between the elevated FeNO group and the normal FeNO group.Results The level of FeNO in the observation group was higher than that in the control group,and the FEV_(1) and FEV_(1)/FVC were lower than those in the control group,and the differences were statistically significant(P<0.05).The complete control rate of asthma in the elevated FeNO group were higher than those in the normal FeNO group,and the difference was statistically significant(P<0.05).After treatment,the FEV_(1),FEV_(1)/FVC and the scores of ACT in the two groups were higher than those before treatment,and the elevated FeNO group was higher than the normal FeNO group,and the differences were statistically significant(P<0.05).Conclusion FeNO is highly expressed in patients with bronchial asthma,the detection of FeNO and pulmonary function indicators can provides guidance for clinical diagnosis,treatment and management of bronchial asthma,and improve the diagnosis,treatment and clinical control effect of bronchial asthma,which has high application value.
