血常规检验对糖尿病肾病早期鉴别诊断效果的影响

Effect of blood routine examination on early differential diagnosis of diabetes nephropathy

目的探讨血常规检验对糖尿病肾病早期鉴别诊断效果的影响。方法选取2019年5月至2020年5月本院收治的168例糖尿病患者作为观察组,根据其24 h尿微量清蛋白排泄率指标不同分为单一糖尿病组(n=41)、早期糖尿病肾病组(n=58)、临床糖尿病肾病组(n=69),同时选取同期于本院健康体检的168名正常人作为对照组。比较观察组与对照组血常规指标及单一糖尿病组、早期糖尿病肾病组与临床糖尿病肾病组血常规指标。结果观察组红细胞分布宽度(RDW)水平明显高于对照组,观察组血红蛋白(Hb)、平均红细胞体积(MCV)水平明显低于对照组(P<0.05);单一糖尿病组、早期糖尿病肾病组、临床糖尿病肾病组随着病情的加重,Hb、MCV水平持续下降,RDW水平持续上升。结论糖尿病患者RDW水平高于健康人,Hb、MCV水平低于健康人,且糖尿病肾病患者的Hb、MCV水平会随病情的加重持续下降,RDW水平会随病情的加重持续上升。因此,血常规检验有助于提高早期糖尿病肾病的检出率,为早期糖尿病肾病患者的病情监测提供有效依据。

Objective To explore the effect of blood routine examination on early differential diagnosis of diabetes nephropathy.Methods 168 patients with diabetes admitted to our hospital from May 2019 to May 2020 were selected as the observation group,and were divided into single diabetes group(n=41),early diabetes nephropathy group(n=58),and clinical diabetes nephropathy group(n=69)according to their 24-hour urinary micro-albumin excretion rate.At the same time,168 normal people who were in the health examination of our hospital were selected as the control group.The blood routine indexes of the observation group and the control group,the single diabetes group,the early diabetes nephropathy group and the clinical diabetes nephropathy group were compared.Results The red cell distribution width(RDW)level in the observation group was significantly higher than that in the control group,and the hemoglobin(Hb)and mean corpuscular volume(MCV)levels in the observation group were significantly lower than those in the control group(P<0.05);With the aggravation of the disease,the levels of Hb and MCV in the single diabetes group,the early diabetes nephropathy group and the clinical diabetes nephropathy group continued to decline,and the level of RDW continued to rise.Conclusion The level of RDW in patients with diabetes is higher than that in healthy people,and the levels of Hb and MCV in patients with diabetes nephropathy are lower than those in healthy people.Therefore,routine blood test can help to improve the detection rate of early diabetes nephropathy,and provide an effective basis for monitoring the condition of early diabetes nephropathy patients.
