米非司酮联合米索前列醇用于计划生育手术的临床疗效

Clinical efficacy of mifepristone combined with misoprostol in family planning surgery

目的观察米非司酮联合米索前列醇用于计划生育手术的临床疗效。方法选取2018年10月至2020年8月于本院妇产科行普通人工流产术终止妊娠的144例早孕患者作为研究对象,按照随机数字表法分为3组,每组48例。A组术前2 d服用米非司酮治疗,B组术前3 h经阴道置入米索前列醇治疗,C组术前联合应用米非司酮与米索前列醇治疗。比较3组宫颈口软化扩张情况、终止妊娠效果、手术时间、术中失血量及并发症发生情况。结果C组宫颈口软化扩张总有效率为100.00%,明显高于A组(85.42%)、B组(87.50%),差异有统计学意义(P<0.05);C组手术时间[(2.69±0.47)min]短于A组[(3.81±0.68)min]及B组[(3.74±0.62)min],术中失血量[(15.90±2.43)ml]少于A组[(21.13±5.06)ml]及B组(19.49±3.58)ml(P<0.05);3组终止妊娠效果、并发症发生率比较差异无统计学意义。结论计划生育手术前采用米非司酮联合米索前列醇可有效促进患者宫颈口软化扩张,显著缩短手术时间,明显减少术中失血量,值得临床推广应用。

Objective To observe the clinical efficacy of mifepristone combined with misoprostol in family planning surgery.Methods 144 cases of early pregnancy who were terminated by general artificial abortion in Department of Obstetrics and Gynecology in our hospital from October 2018 to August 2020 were selected as the research subjects,and they were were divided into three groups according to the random number table,with 48 cases in each group.Group A was treated with mifepristone 2 d before operation,group B was treated with misoprostol through vagina 3 h before operation,and group C was treated with mifepristone and misoprostol before operation.The softening and dilation of cervix,the effect of termination of pregnancy,operation time,intraoperative blood loss and complications were compared among the three groups.Results The total effective rate of softening and dilation of cervical orifice in group C was 100.00%,which was significantly higher than that in the group A(85.42%)and group B(87.50%)(P<0.05);the operation time of group C([2.69±0.47]min)was shorter than that of group A([3.81±0.68]min)and group B([3.74±0.62]min),and the intraoperative blood loss([15.90±2.43]ml)was less than that of group A[(21.13±5.06)ml]and group(B[19.49±3.58]ml)(P<0.05);there was no significant difference in the effect of termination of pregnancy and the incidence of complications among the three groups.Conclusion The use of mifepristone combined with misoprostol before family planning surgery can effectively promote the softening and dilation of cervix,significantly shorten the operation time,and significantly reduce intraoperative blood loss,which is worthy of clinical application.
