胸腔镜手术联合术后循环胸腔热灌注对脓胸患者肺功能的影响研究

摘要 目的研究胸腔镜手术联合术后循环胸腔热灌注对脓胸患者肺功能的影响。方法选取2018年3月至2021年3月本院收治的64例脓胸患者作为研究对象,采用随机数字表法分为对照组和实验组,各32例。对照组采用开放手术治疗,实验组采用胸腔镜手术同时术后联合循环胸腔热灌注治疗。比较两组手术情况、肺功能情况、炎症因子水平。结果实验组手术时间、引流管置管时间均短于对照组,手术出血量、胸腔引流量均低于对照组(P<0.05);治疗后,实验组功能残气量(FRC)、用力肺活量(FVC)、每分钟最大通气量(MVV)高于对照组(P<0.05),实验组降钙素原(PCT)、C反应蛋白(CRP)水平低于对照组(P<0.05)。结论胸腔镜手术联合术后循环胸腔热灌注治疗脓胸能有效改善患者肺功能,减轻炎症反应程度,缩短手术时间,且创伤小,值得临床推广应用。 Objective To investigate the effect of thoracoscopic surgery combined with postoperative circulating thoracothorax thermal perfusion on pulmonary function in patients with empyema.Methods A total of 64 patients with empyema admitted to our hospital from March 2018 to March 2021 were selected as the research subjects,and they were divided into control group and experimental group by random number table method,with 32 cases in each group.The control group was treated with open surgery,and the experimental group was treated with thoracoscopic surgery combined with postoperative circulating thoracothermal perfusion.The operation,pulmonary function and inflammatory factors were compared between the two groups.Results The operation time and drainage tube placement time of the experimental group were shorter than those of the control group,and the operation blood loss and thoracic drainage volume were lower than those of the control group(P<0.05).After treatment,the functional residual capacity(FRC),forced vital capacity(FVC)and minute ventilation volume(MMV)of the experimental group were higher than those of the control group(P<0.05),and the level of procalcitonin(PCT)and C-reactionprotein(CRP)of the experimental group were lower than those of the control group(P<0.05).Conclusion Thoracoscopic surgery combined with postoperative circulating pleural heat perfusion in the treatment of empyema can effectively improve the pulmonary function of patients,reduce the degree of inflammation,shorten the operation time,and less trauma,which is worthy of clinical application.
