自锁托槽矫治对正畸治疗患者牙周组织状况及龈沟液中炎症因子水平的影响

Effect of self-locking bracket on periodontal tissue and inflammatory factors in gingival crevicular fluid of orthodontic patients

目的探讨自锁托槽矫治对正畸治疗患者牙周组织状况及龈沟液中炎症因子水平的影响。方法选取2018年6月至2020年6月本院收治的牙周病患者78例作为研究对象,采用随机数字表法分为对照组和观察组,各39例。两组均进行正畸治疗,对照组行传统托槽矫治器,观察组予以自锁托槽矫治。比较两组牙周组织[探诊深度(PD)、牙龈指数(GI)、菌斑指数(PLI)、龈沟出血指数(SBI)]状况及龈沟液中炎症因子。结果治疗后,观察组PD(2.31±0.39)mm、GI(1.08±0.59)、PLI(1.47±0.41)、SBI(1.79±0.44),均低于对照组的(2.86±0.52)mm、(1.71±0.57)、(1.92±0.48)、(2.35±0.61),观察组IL-1β(22.82±1.84)μg/L、TNF-ɑ(2.02±0.14)μg/L、IL-6(1.76±0.43)μg/L,均低于对照组的(29.09±2.63)μg/L、(2.69±0.33)μg/L、(2.37±0.51)μg/L,差异有统计学意义(P<0.05)。结论正畸治疗患者采用自锁托槽矫治效果更好,对牙周组织的刺激更小,改善牙周状况,降低炎症反应,值得临床推广应用。

Objective To investigate the effects of self-locking brackets on periodontal tissue and inflammatory factors in gingival crevicular fluid(GCF)of orthodontic patients.Methods 78 patients with periodontal disease admitted to our hospital from June 2018 to June 2020 were selected as the research subject,and they were divided into control group and observation group by random number table method,with 39 cases in each group.Both groups received orthodontic treatment,the control group received traditional bracket appliance,and the observation group received selflocking bracket.The periodontal tissues(PD,GI,PLI,SBI)and inflammatory factors in gingival crevicular fluid were compared between the two groups.Results After treatment,PD(2.31±0.39)mm,GI(1.08±0.59),PLI(1.47±0.41),SBI(1.79±0.44)in the observation group were lower than those in the control group(2.86±0.52)mm,(1.71±0.57),(1.92±0.48),(2.35±0.61),and IL-1βin the observation group was(22.82±1.84)μg/L,TNFɑ(2.02±0.14)μg/L and IL-6(1.76±0.43)μg/L were lower than those in the control group(29.09±2.63)μg/L,(2.69±0.33)μg/L and(2.37±0.51)μg/L,the difference was statistically significant(P<0.05).Conclusion The self-locking bracket has a better effect on orthodontic patients,less irritation to periodontal tissue,and can improve periodontal condition and reduce inflammatory reaction,which is worthy of clinical application.
