高强度聚焦超声消融治疗对子宫肌瘤患者子宫血流动力学指标、卵巢功能的影响

Effects of high-intensity focused ultrasound ablation on uterine hemody-namics and ovarian function of patients with uterine fibroids

目的探讨高强度聚焦超声消融治疗对子宫肌瘤患者子宫血流动力学指标、卵巢功能的影响。方法选取2019年7月至2021年7月收治的70例子宫肌瘤患者,以随机数字表法将其分为对照组和观察组,每组35例。对照组采用常规子宫肌瘤切除术治疗,观察组采用高强度聚焦超声消融治疗。比较两组的治疗效果。结果治疗后,观察组的子宫内膜、子宫内膜下血流指数(FI)、血管化指数(VI)及血管化血流指数(VFI)均高于对照组(P<0.05)。治疗后,观察组的促卵泡生长激素(FSH)、促黄体生长素(LH)、雌二醇(E2)水平低于对照组(P<0.05)。治疗后,观察组的子宫内膜厚度、容积均大于对照组(P<0.05)。治疗后,观察组的改良Kupperman评分低于对照组,生活质量综合评定问卷(GQOLI-74)评分高于对照组(P<0.05)。结论高强度聚焦超声消融治疗子宫肌瘤患者有助于进一步改善子宫血流动力学和卵巢功能,缓解疾病症状,提高生活质量。

Objective To investigate the effects of high-intensity focused ultrasound ablation on uterine hemodynamics and ovarian function of patients with uterine fibroids.Methods Seventy patients with uterine fibroids admitted from July 2019 to July 2021 were divided into control group and observation group by random number table method,with 35 cases in each group.The control group was treated with routine myomectomy,and the observation group was treated with high-intensity focused ultrasound ablation.The therapeutic effects of the two groups were compared.Results After treatment,flow index(FI),vascularization index(VI)and vascularization-flow index(VFI)of endometrium and subendometrium in the observation group were higher than those in the control group(P<0.05).After treatment,the levels of follicle stimulating hormone(FSH),luteinizing hormone(LH)and estradiol(E2)in the observation group were lower than those in the control group(P<0.05).After treatment,the endometrial thickness and volume in the observation group were larger than those in the control group(P<0.05).After treatment,the modified Kupperman score in the observation group was lower than that in the control group,and the Generic Quality of Life Inventory-74(GQOLI-74)score was higher than that in the control group(P<0.05).Conclusion High-intensity focused ultrasound ablation for patients with uterine fibroids is helpful to further improve uterine hemodynamics and ovarian function,alleviate disease symptoms and improve quality of life.
