射频消融联合TACE对原发性肝癌患者血清甲胎蛋白、基质金属蛋白酶-9水平的影响

Effects of radiofrequency ablation combined with TACE on serum alpha-fetoprotein and matrix metalloproteinase-9 levels in patients with primary liver cancer

目的探讨射频消融联合经导管动脉化疗栓塞术(TACE)对原发性肝癌患者血清甲胎蛋白(AFP)、基质金属蛋白酶-9(MMP-9)水平的影响。方法选取2018年4月至2021年4月我院收治的76例原发性肝癌患者作为研究对象,采用随机数字表法将其分为对照组和观察组,各38例。对照组采用TACE治疗,观察组在此基础上联合射频消融治疗。比较两组的治疗效果。结果观察组的治疗总有效率高于对照组(P<0.05)。治疗后,观察组的天门冬氨酸氨基转移酶(AST)、丙氨酸转氨酶(ALT)及总胆红素(TBIL)水平均低于对照组(P<0.05)。治疗后,观察组的AFP、MMP-9水平均低于对照组(P<0.05)。治疗后,观察组的白细胞分化抗原4阳性(CD4^(+))、白细胞分化抗原4阳性/白细胞分化抗原8阳性(CD4^(+)/CD8^(+))均高于对照组,白细胞分化抗原8阳性(CD8^(+))低于对照组(P<0.05)。结论射频消融联合TACE治疗原发性肝癌效果显著,能改善患者肝功能、免疫功能,下调AFP、MMP-9水平。

Objective To investigate the effects of radiofrequency ablation combined with transcatheter arterial chemoembolization(TACE)on serum alpha-fetoprotein(AFP)and matrix metalloproteinase-9(MMP-9)levels in patients with primary liver cancer.Methods A total of 76 patients with primary liver cancer admitted in our hospital from April 2018 to April 2021 were selected as the research objects.The patients were divided into control group and observation group by random number table method,with 38 cases in each group.The control group was treated with TACE,and the observation group was treated with radiofrequency ablation on this basis.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the levels of aspartate aminotransferase(AST),alanine aminotransferase(ALT)and total bilirubin(TBIL)in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of AFP and MMP-9 in the observation group were lower than those in the control group(P<0.05).After treatment,the cluster of differentiation 4 positive(CD4^(+))and cluster of differentiation 4 positive/cluster of differentiation 8 positive(CD4^(+)/CD8^(+))in the observation group were higher than those in the control group,and the cluster of differentiation 8 positive(CD8^(+))was lower than that in the control group(P<0.05).Conclusion Radiofrequency ablation combined with TACE in the treatment of primary liver cancer has a significant effect,which can improve liver function and immune function of patients,and down-regulate AFP and MMP-9 levels.
