普通针刺联合中医康复训练治疗缺血性脑卒中后吞咽障碍的临床效果

Clinical Effect of Dysphagia after Ischemic Stroke by Common Acupuncture Combined with Rehabilitation Training of Traditional Chinese Medicine

目的分析普通针刺联合中医康复训练对脑卒中后伴有吞咽功能障碍的治疗效果。方法选择2019年1月至2022年6月我院康复科收治88例脑卒中后合并吞咽功能障碍者作为研究对象,将其随机分为对照组及观察组,各44例。观察组采用针刺结合康复训练,对照组仅采用康复训练,均为每日1次,每周5次,共治疗4周。比较两组治疗前后洼田饮水试验分级、标准吞咽功能评价量表(SSA)评分、总有效率。结果两组患者治疗前饮水试验分级对比经Mann-Whitney U检验后证实差异有统计学意义,P<0.05,治疗后两组患者饮水试验分级差异无统计学意义,P>0.05。治疗后两组患者SSA评分与治疗前相比显著下降(P<0.05)。在疗程结束后,观察组患者总有效率为93.18%,对照组为81.82%,差异有统计学意义(P<0.05)。结论应用普通针刺联合中医康复训练治疗缺血性脑卒中后吞咽障碍的效果确切。

Objective To analyze the therapeutic effect of common acupuncture combined with traditional Chinese medicine rehabilitation training on swallowing dysfunction after stroke.Methods From January 2019 to June 2022,88 patients with swallowing dysfunction after stroke admitted to the Department of Rehabilitation our hospital were selected as research subjects,and they were randomly divided into a control group and an observation group,with 44 cases in each group.The observation group used acupuncture combined with rehabilitation training,while the control group only used rehabilitation training,both once a day and five times a week,for a total of 4 weeks of treatment.The grading of the Wada drinking water test,the score of the standard swallowing function assessment scale(SSA),and the total effective rate were compared between the two groups before and after treatment.Results There was a significant statistical difference between the two groups in the grading of drinking water test before treatment,which was confirmed by Mann Whitney U test(P<0.05).After treatment,there was no significant statistical difference between the two groups in the grading of drinking water test,with P>0.05.After treatment,the SSA scores of patients in the two groups decreased significantly compared with those before treatment(P<0.05).After the treatment,the total effective rate of patients in the treatment group was 93.18%,compared to 81.82%in the control group,there was a significant difference in the efficacy between the two groups(P<0.05).Conclusion The application of ordinary acupuncture combined with traditional Chinese medicine rehabilitation training in the treatment of swallowing disorders after ischemic stroke has a definite effect.
