Symptom Burden and TCM Symptom Group Analysis of Patients with Head and Neck Tumor After Concurrent Radiotherapy and Chemotherapy

目的:分析头颈部肿瘤患者结束同步放化疗时症状负担和中医症状群的分布。方法:收集2014年10月至2016年5月国内11家三甲医院接受同步放化疗的113例头颈部肿瘤患者的临床资料并进行数据分析。采用肿瘤常见症状及中医症状调查表(M.D.Anderson symptom inventory for traditional Chinese medicine,MDASI-TCM)进行症状评估。描述统计及探索性因子分析探讨症状负担及中医症状群分布。结果:在113例头颈部肿瘤患者结束同步放化疗时,发生率前5位的症状分别是疼痛(98%)、疲劳(96%)、口干(89%)、苦恼(87%)、胃口差(85%);分析后得出3个中医症状群,分别为气阴两虚相关症状群、热毒阴伤相关症状群、情感相关症状群。结论:头颈部肿瘤患者结束同步放化疗时症状发生频率高、程度重,存在多个中医症状群。

Objective:To analyze the distribution of symptom burden and TCM symptom groups in patients with head and neck cancer after concurrent radiotherapy and chemotherapy.Methods:From October 2014 to May 2016,the clinical data of 113 patients with head and neck cancer who received concurrent radiotherapy and chemotherapy in 11 domestic 3A hospitals were collected and analyzed.M.D.Anderson Symptom Questionnaire for Traditional Chinese Medicine(MDASI-TCM)was used to evaluate the symptoms.Descriptive statistics and exploratory factor analysis were used to explore the burden of symptoms and the distribution of TCM syndrome groups.Results:When 113 patients with head and neck tumor finished concurrent radiotherapy and chemotherapy,the top five symptoms were pain(98%),fatigue(96%),dry mouth(89%),distress(87%)and poor appetite(85%).After the analysis,three TCM symptom groups are obtained,which are related to deficiency of both qi and yin,toxic heat and yin injury and emotion.Conclusion:Patients with head and neck tumors have high frequency and severe symptoms when they finish concurrent radiotherapy and chemotherapy,and there are many TCM symptom groups.
