尿毒症维持性血液透析患者疲乏现状及其影响因素

Fatigue status and its influencing factors in uremic patients undergoing maintenance hemodialysis

目的:分析尿毒症维持性血液透析(MHD)患者疲乏现状及其影响因素。方法:回顾性分析2021年1月至2022年1月该院收治的106例尿毒症MHD患者的临床资料,采用Piper疲乏量表(PFS)评价患者疲乏现状,进行单因素分析,并采用多元线性回归分析尿毒症MHD患者疲乏的影响因素。结果:106例尿毒症MHD患者平均PFS评分为(5.24±1.01)分,处于中度疲乏;不同性别、文化程度、家庭人均月收入、透析时间、运动情况、透析并发症、疾病不确定感量表(MUIS-A)评分、家庭关系患者的PFS评分比较,差异均有统计学意义(P<0.05);不同年龄、民族、婚姻状况、居住地、职业、饮酒和吸烟状况患者的PFS评分比较,差异均无统计学意义(P>0.05);多元线性回归分析结果显示,性别、透析时间、运动情况、透析并发症、MUIS-A评分均为尿毒症MHD患者疲乏的危险因素,家庭关系为尿毒症MHD患者疲乏的保护因素(P<0.05)。结论:性别、透析时间、运动情况、透析并发症、MUIS-A评分均为尿毒症MHD患者疲乏的危险因素,家庭关系为尿毒症MHD患者疲乏的保护因素。

Objective:To investigate fatigue status and its influencing factors in uremic patients undergoing maintenance hemodialysis(MHD).Methods:The clinical data of 106 uremic patients undergoing MHD admitted to the hospital from January 2021 to January 2022 were retrospectively analyzed.The Piper fatigue scale(PFS)was used to evaluate the fatigue status of these patients.The univariate analysis was performed.The multivariate linear regression equation was used to analyze the influencing factors of fatigue in the uremic patients undergoing MHD.Results:The average PFS score of the 106 uremic patients undergoing MHD was(5.24±1.01)points,which was in moderate fatigue.There were statistically significant differences in the PFS scores among the patients with different genders,educational levels,family per capita monthly income,dialysis time,exercise status,dialysis complications,Mishel uncertainty in illness scale(MUIS-A)scores,and family relationships(P<0.05).There was no statistically significant difference in the PFS scores among patients with different ages,ethnic groups,marital status,place of residence,occupation,alcohol consumption,and smoking status(P>0.05).Multivariate linear regression analysis showed that gender,dialysis time,exercise status,dialysis complications and MUIS-A score were all risk factors for fatigue in the uremic patients undergoing MHD,and family relationship was a protective factor(P<0.05).Conclusions:Gender,dialysis time,exercise status,dialysis complications and MUIS-A score are the risk factors for fatigue in the uremic patients undergoing MHD,and family relationship is the protective factor.
