血清sFRP5、LCN-2水平与急性左心衰竭患者心功能分级及预后不良的相关性

Correlations among serum sFRP5,LCN-2 levels and cardiac function classification and poor prognosis in patients with acute left heart failure

摘要 目的:分析血清分泌型卷曲相关蛋白5(sFRP5)、脂质运载蛋白-2(LCN-2)水平与急性左心衰竭(ALHF)患者心功能分级及预后不良的相关性。方法:回顾性分析2018年1月至2022年10月该院收治的70例ALHF患者的临床资料,纳入观察组。另收集同期该院70名健康体检者的资料,纳入对照组。两组均进行血清s FRP5、LCN-2水平检测,并对观察组随访3个月,统计预后情况。比较两组血清s FRP5、LCN-2水平,比较不同美国纽约心脏病协会(NYHA)心功能分级和不同预后患者血清s FRP5、LCN-2水平,并采用点二列相关性分析血清s FRP5、LCN-2水平与ALHF患者心功能分级及预后不良的相关性。结果:观察组血清s FRP5水平低于对照组,LCN-2水平高于对照组,差异有统计学意义(P<0.05);NYHA心功能Ⅳ级患者血清s FRP5水平低于Ⅲ级患者,LCN-2水平高于Ⅲ级患者,差异有统计学意义(P<0.05);预后不良患者血清s FRP5水平低于预后良好患者,LCN-2水平高于预后良好患者,差异有统计学意义(P<0.05);点二列相关性分析结果显示,血清s FRP5水平与ALHF患者NYHA心功能分级呈负相关(r<0,P<0.05),与预后不良呈正相关(r>0,P<0.05);LCN-2水平与NYHA心功能分级呈正相关(r>0,P<0.05),与预后不良呈负相关(r<0,P<0.05)。结论:血清sFRP5水平与ALHF患者NYHA心功能分级呈负相关,与预后不良呈正相关;LCN-2水平与ALHF患者NYHA心功能分级呈正相关,与预后不良呈负相关。 Objective:To analyze correlations among serum secreted frizzled-related protein 5(sFRP5),lipocalin-2(LCN-2)levels and cardiac function classification and poor prognosis in patients with acute left heart failure(ALHF).Methods:The clinical data of 70 patients with ALHF admitted to the hospital from January 2018 to October 2022 were retrospectively analyzed and they were included in observation group.The data of 70 healthy subjects in the hospital during the same period were collected and they were included in control group.The serum sFRP5 and LCN-2 levels were detected in both groups.The observation group was followed up for 3 months.The prognosis was statistically analyzed.The levels of serum sFRP5 and LCN-2 were compared between the two groups.The levels of serum sFRP5 and LCN-2 in the patients with different NYHA cardiac function classifications and different prognosis were compared.The correlations among the serum sFRP5,LCN-2 levels and cardiac function classification and poor prognosis in the patients with ALHF were analyzed by point biserial correlation.Results:The serum sFRP5 level in the observation group was lower than that in the control group,the LCN-2 level was higher than that in the control group,and the differences were statistically significant(P<0.05).The level of serum sFRP5 in the patients with NYHA cardiac function classification IV was lower than that in the patients with classification III,the level of LCN-2 was higher than that in the patients with classification III,and the differences were statistically significant(P<0.05).The level of serum sFRP5 in the patients with poor prognosis was lower than that in the patients with good prognosis,the level of LCN-2 was higher than that in the patients with good prognosis,and the differences were statistically significant(P<0.05).The point biserial correlation analysis showed that the serum sFRP5 level was negatively correlated with the NYHA cardiac function classification in the ALHF patients(r<0,P<0.05)and positively correlated with poor prognosis(r>0,P<0.05);while the LCN-2 level was positively correlated with the NYHA cardiac function classification(r>0,P<0.05)and negatively correlated with poor prognosis(r<0,P<0.05).Conclusions:The serum sFRP5 level is negatively correlated with the NYHA cardiac function classification in the ALHF patients,and positively correlated with poor prognosis;while the LCN-2 level is positively correlated with the NYHA cardiac function classification and negatively correlated with poor prognosis.
