内镜下冷圈套器息肉切除术与常规息肉切除术治疗结肠息肉患者的效果比较

Comparison of effects of endoscopic cold snare polypectomy and conventional polypectomy on patients with colonic polyps

目的:比较内镜下冷圈套器息肉切除术(CSP)与常规息肉切除术治疗结肠息肉患者的效果。方法:选取2019年6月至2020年6月该院收治的94例结肠息肉患者进行前瞻性研究,按照随机数字表法将其分为观察组与对照组各47例。对照组行内镜下常规息肉切除术治疗,观察组行内镜下CSP治疗。比较两组息肉切除时间、住院时间、息肉完整切除率,术前和术后1 d血清炎性因子[C反应蛋白(CRP)、白细胞介素-6(IL-6)]水平,术后1、2、3 d疼痛程度[视觉模拟评分法(VAS)]评分,手术前后日常生活活动能力[Barthel指数(BI)]评分,以及并发症发生率。结果:观察组息肉切除时间、住院时间均短于对照组,差异有统计学意义(P<0.05);观察组息肉完整切除率和BI评分均高于对照组,差异有统计学意义(P<0.05);观察组术后1 d的IL-6、CRP水平和术后1、2、3 d的VAS评分均低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为2.13%(1/47),低于对照组的17.02%(8/47),差异有统计学意义(P<0.05)。结论:内镜下CSP治疗结肠息肉患者可缩短息肉切除时间、住院时间,提高息肉完整切除率和BI评分,降低炎性因子水平、VAS评分和并发症发生率,效果优于内镜下常规息肉切除术治疗。

Objective:To compare effects of endoscopic cold snare polypectomy(CSP)and conventional polypectomy in treatment of colon polyps.Methods:A prospective study was conducted on 94 patients with colon polyps admitted to the hospital from June 2019 to June 2020.They were divided into observation group and control group according to the random number table method,47 cases in each.The control group was treated with endoscopic conventional polypectomy,while the observation group was treated with endoscopic CSP.The polyp resection time,the hospitalization time,the complete polyp resection rate,the levels of inflammatory factors[C-reactive protein(CRP),interleukin-6(IL-6)]before and 1 day after the surgery,the pain degree[visual analogue scale(VAS)]1,2 and 3 days after the surgery,the activities of daily living scores[Barthel index(BI)]before and after the surgery,and incidence of complications were compared between the two groups.Results:The polyp resection time and the hospitalization time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).The complete resection rate and the BI score in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of IL-6 and CRP at 1 d after the surgery and the VAS scores 1,2 and 3 d after the surgery in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications of the observation group was 2.13%(1/47),which was lower than 17.02%(8/47)in the control group,and the difference was statistically significant(P<0.05).Conclusions:Endoscopic CSP in the treatment of the patients with colonic polyps can shorten the polyp resection time and the hospitalization time,improve the complete polyps resection rate and the score of BI,and reduce the levels of inflammatory factors,the VAS scores and the complication rate.Moreover,it is superior to conventional endoscopic polypectomy.
