降逆益中汤加减联合常规西药治疗非糜烂性胃食管反流病中虚气逆证合并焦虑抑郁患者的效果

Effects of Jiangni Yizhong decoction combined with conventional Western medicine in treatment of patients with non-erosive gastroesophageal reflux disease with syndrome of reversed flow of qi complicated with anxiety and depression

目的:观察降逆益中汤加减联合常规西药治疗非糜烂性胃食管反流病(NERD)中虚气逆证合并焦虑抑郁患者的效果。方法:选取2019年10月至2022年10月该院收治的118例NERD中虚气逆证合并焦虑抑郁患者进行前瞻性研究,按照随机数字表法将其分为对照组和观察组各59例。对照组予以常规西药治疗,观察组在对照组基础上联合降逆益中汤加减治疗。比较两组临床疗效,治疗前后汉密尔顿焦虑量表(HAMA)评分、17项汉密尔顿抑郁量表(HAMD-17)评分、中医证候积分,以及不良反应发生率。结果:观察组治疗总有效率为96.61%(57/59),高于对照组的77.97%(46/59),差异有统计学意义(P<0.05);治疗后,观察组HAMA、HAMD-17评分和中医证候积分均低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:降逆益中汤加减联合常规西药治疗NERD中虚气逆证合并焦虑抑郁患者,可提高治疗总有效率,降低焦虑、抑郁评分和中医证候积分,效果优于单纯常规西药治疗。

Objective:To observe effects of Jiangni Yizhong decoction combined with conventional Western medicine in treatment of patients with non-erosive gastroesophageal reflux disease(NERD)with syndrome of reversed flow of qi complicated with anxiety and depression.Methods:From October 2019 to October 2022,118 patients with NERD with syndrome of reversed flow of qi complicated with anxiety and depression were selected for the prospective study.According to the random number table method,they were divided into control group and observation group,59 cases in each group.The control group was treated with conventional Western medicine,while the observation group was treated with Jiangni Yizhong decoction on the basis of that of the control group.The clinical efficacy,the Hamilton anxiety scale(HAMA)score,the 17-item Hamilton depression scale(HAMD-17)score,the TCM syndrome score,and the incidence of adverse reactions were compared between the two groups before and after the treatment.Results:The total effective rate of treatment in the observation group was 96.61%(57/59),which was higher than 77.97%(46/59)in the control group,and the difference was statistically significant(P<0.05).After the treatment,the scores of HAMA,HAMD and TCM syndrome of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Jiangni Yizhong decoction combined with conventional Western medicine can improve the total effective rate of treatment and reduce the anxiety and depression scores and the TCM syndrome scores in the treatment of the NERD patients with syndrome of reversed flow of qi combined with anxiety and depression.Moreover,it is superior to single conventional Western medicine treatment.
